GENERAL PURPOSE OF THE JOBWe need this job because we help minimize risk to the Village by releasing medical records supporting services provided to our patients. This position reports to the Assistant Health Information Manager. This position performs release of PHI (Protected Health Information) and medical records compilation relative to Government/Payor Audit, Patient, and Third-Party Requests for ROI (Release of Information) according to established policies, regulations and Federal/State laws. Position fosters respect for patient privacy by maintaining confidentiality in all aspects of role. Position promotes positive customer relations through prompt and courteous service in this fast-paced and high volume environment.ESSENTIAL FUNCTIONSGovernment / Payor Audits
Independently prepare medical record packet in response to RAC, CERT, ADR and other government audit requests
Assess the appropriateness and accuracy of medical record documentation to support accurate assignment of ICD-10, CPT, HCPCS and other third party payer codes
Proficient with clinical applications to print and collate Designated Record Set
Independently compile appeal communication to Fiscal Intermediary as appropriate based upon appeals and overall documentation findings
Contact facilities to obtain hard copy medical records to support charges
Release of Information
Access active and archived EHR systems to produce electronic medical records for any DOS for all categories of requests including internal, ESRD Network and other Government Investigation requests
Independently prepare and release medical and billing information in compliance with Joint Commission, 50-state laws and company policies
Proficient in collating medical records into Designated Record Set
Identifying and reporting deficiencies and discrepancies in the medical record to the Manager of HIM
HIPAA Review and LoggingUnderstand and apply HIPAA law; correctly approve or deny request with minimal errorAccurately assigns requests to correct CategoryProactively contacts requestors as neededClearly and concisely communicate to internal and external customersKnowledge Medical Record Policy and responds to queries by phone or emailOt her duties and responsibilities as assigned, not limited to
Consistent, regular, punctual attendance is required
Attend team meetings, phone conferences and training as needed.
Work overtime with little or no notice as needed
