Develops, refines, implements, and communicates all clinical quality improvement activities, and objectives. Acts as subject matter expert to ensure quality programs will meet/exceed guidelines. Using available data analytics, responsible for monitoring and reporting results of quality efforts and providing guidance to the field and senior leadership team on matters relating to progress. Promotes commitment to corporate standards, codes of conduct and ensures compliance with local, federal and Medicare laws and requirements.

Manages and directs quality services while promoting company-wide understanding, communication, and coordination of quality standards.

Develops, implements and refines the comprehensive processes of the quality program to meet the demographic needs of the population served, key stakeholders, and partners.

Analyzes and reports patient care data and develops plans and programs to support continuous quality improvement.

Works collaboratively with key corporate and field leaders to identify opportunities for improvement, trend analysis, and education that results in action plans for problem resolution.

Represents quality programs by facilitating and participating in assigned committees and workgroups.

Educates and communicates within the company matrix on risk adjustment, quality measurement, and clinical documentation requirements.

Supervises Quality Support Specialists to identify opportunities for collaboration and facilitates development and follow up of action plans or deliverables.

Attends national, regional and local meetings, as requested.

Promotes commitment to Brookdale standards, systems, policies, and procedures. Educates associates regarding corporate commitment to all laws, regulations, and guidelines of the company, state, and federal programs. Adheres to established confidentiality standards and promotes the standards are maintained by all regional office and field level associates.

Minimum five years of home health experience and three years of management experience required; or equivalent combination of education and experience.Certifications, Licenses, and Other Special RequirementsCurrent state licensure and / or minimum qualifications required for a nursing. Current OASIS and Coding certifications required.Physical Demands and Working Conditions
