When not in the field, you will work from your home office. The area we cover is Shelby County, Tipton, and Fayette County. It is preferred that you reside in this area but we will consider candidates who reside close enough to travel within these counties daily.The LTSS Service Coordinator is responsible for providing ongoing coordination of ECF CHOICES services and supports with a focus on supporting individuals to explore and obtain competitive integrated employment and maximize community integration. This role supports the individual's person centered support plan to meet their specific needs, with the goal of optimizing health care and promoting and supporting integrated, competitive employment and independent living as the first and preferred option for all individuals with I/DD. Responsibilities include, but are not limited to the following:

Performs telephonic or face-to-face contacts at required intervals, for the identification, evaluation, coordination and management of person's needs.

Obtain clinical data as needed.

Complete Initial and Annual Assessments within time frames set forth by CRA and Amerigroup.

Assess Individual needs for ECF Services including Level of Care changes at enrollment and annually for each person on caseload.

Identifies members with potential clinical health care needs (including, but not limited to, potential for high risk complications) and refers those members' cases to the clinical healthcare management team. Consult with RN Support Coordinator(s) within each region for additional information on health care conditions and /or management.

Manages non-clinical needs of members with chronic illnesses, co-morbidities, and/or disabilities, to insure cost effective and efficient utilization of long term services and supports.

Obtain a thorough and accurate medical, psychosocial and behavioral history, with clearly defined preferences, dislikes and support needs, to develop a person-centered support plan.

Assist the person supported in establishing short and long term goals through the development of the person centered support plan, in collaboration with natural support.

Empower people supported to fully engage in the planning process to drive ECF services and goals that are important to/for the person supported.

May also serve as mentor or preceptor for new staff, assisting in the formal training of associates and may be involved in process improvement initiatives.

Requires:

BA/BS in a health related field; 2 years of experience working with a social work agency; or any combination of education and experience, which would provide an equivalent background.

Experience with Individuals with chronic illnesses, co-morbidities, and/or disabilities in a Service Coordinator, Case Management, Advocate or similar role strongly preferred.

Strong computer skills to include Excel and Outlook.

Experience in Long Term care desired.

Approximately 75% travel required within assigned territory.

Experience working with the Intellectually/Developmentally Disabled population preferred.

Knowledge of ECF Choices and Medicaid programs a plus!
