Omni Community Health is seeking a Family Advocate for the East and Smoky regions of the Relative Caregiver Program. Full-time Family Advocate needed for Relative Caregiver Program (RCP) in East TN. Responsible for several counties throughout East Tennessee. The RCP mission is to provide a comprehensive local support network promoting family stability and to ensure the continued successful development of children who are in the primary care of relatives, and other related family. Minimum requirement- bachelor's degree in social services or related field and one year professional experience working with children and families required. Job requirements include managing referrals and caseload by explaining RCP program to interested parties and conducting in home assessments of family strengths and needs; developing family support plans and empowering RCP families to seek and obtain needed supports & services; conducting support groups and enrichment activities for relative children, caregivers and birth families; advocating for the RCP families being served and providing information and referral for East Tennessee area resources; collaboration and advocacy with courts, schools and private and public agencies such as DCS/ DHS; and an ability to document casework activities and capture key program data and outcomes and report those to the RCP Program Director on a weekly basis. Weekly travel (reimbursed) and basic computer skills necessary. The Relative Caregiver Program is a program of Omni Community Health.
