Reviews electronic medical records and identifies all treated diagnoses and significant procedures performed.

Sequences diagnoses and procedures according to definition of principal diagnosis, other co-morbid conditions and complications, and according to definition of principal procedure and other procedures.

Uses 3M 360 Encompass to assign ICD-10-CM diagnosis and procedure codes, CPT codes and modifiers when indicated, to ensure coding accuracy and APC accuracy.

Utilizes online coding references, Local Coverage Determinations (LCDs), Medi-Bulletins and other payor guidelines to ensure appropriate code assignment for Compliance, Billing, and Medical Necessity.

Transfers diagnosis and procedure codes and modifiers from 3M 360 to Cerner AccessHIM and through the interface to patient abstracts in McKesson Series AS400.

Enters information into the AS400 Abstract Maintenance Program as indicated. (Example: physician ID numbers, consultants, discharge disposition, etc.)

Performs physician queries as indicated for clarification of diagnoses, procedures, and conflicting information for appropriate coding assignment.

Enters diagnosis codes and physician ID's for hospital-based physician services in AS400 1500 DR/Diagnosis Maintenance. Selects diagnosis code appropriate to the service or procedure performed. Responsible for the coding and optimization of electronic medical records. Under the direct supervision of the Coding Manager, responsible for assigned 8-hour shift, 5 days a week. Clinic Payer Account Receivable knowledge, preferred.Skill and proficiency in diagnosis and procedure coding, and other principles, concepts and techniques of Health Information Management. Such proficiency is acquired through certification accredited by AHIMA.Current certification as CCA , RHIT, RHIA, or CCS by AHIMA or obtained within the first year of employment. Knowledge of Health Information Management practices, coding and coding guidelines, as acquired through CCA certification. Experience in coding diagnostic outpatients, emergency department records, ancillary outpatients or physician office coding required.
