Making enrollment calls to members who are eligible for telephonic wellness programs.

Assessing health information for members with various coaching needs that may include chronic conditions, lifestyle management, weight management, and smoking cessation.

Setting up goals and providing coaching as well as telephonic follow up calls to members to continue to work on health goals.

Encourage members to get connected to resources that are available to them through their insurance, which could include participation in additional or different coaching programs

Here are just a few ways in which your clinical knowledge and expertise may be utilized:

Utilize motivational interviewing skills to provide health coaching for members with chronic conditions or other identified health needs.

Implementing and/or coordinating coaching care plans and discussing clinical health recommendations and preventative recommendations.

Assist with increasing participant's motivational level by addressing barriers and assessing needs.

The Health Educator is responsible for responding to inbound inquiries for disease management or medication alert programs.Primary duties may include but are not limited to:

Provides information to members eligible for disease management or medication alert programs.

Makes outbound attempts to program members to provide health information according to program standards.

Receives or places calls to members who have failed to enroll with disease management to encourage their enrollment in the correct disease management program.

Diffuses situations with members who are resistant to health coaching and/or disease management interventions.

Provides consultation to department associates regarding methods/approaches to help members recognize and overcome barriers to improve their health.

Assists with increasing participants motivational level by addressing barriers and assessing needs.

Focuses on improving member health by promoting incremental behavior change leading to healthier outcomes.

Conducts behavioral assessments to identify individual member knowledge skills and behavioral needs.

May coordinate with others in the multi-disciplinary clinical team.

Qualifications

Requires BA/BS in appropriate field of specialization;

2 years of related experience in direct care health education exercise instruction or patient education;

Certification or advanced certification and/or license appropriate to field of specialty is preferred;

Or any combination of education and experience, which would provide an equivalent background.

Prior experience in health coaching in a call center environment and/or patient education and behavior change techniques and strong understanding of disease management principles is preferred.

Multi-state licensure preferred in accordance with company policy preferred.

- Medical Health coaching with adult population is strongly preferred.

PC skills within a windows environment preferred.

Proficient with Microsoft Excel, Outlook, Power Point and Word preferred.

Prior call center or telephonic health education/clinical coaching experience a plus.

Strong communication skills preferred.

CHES certification an added plus.

Bi-lingual English/Spanish a plus.
