Job Description

Registered Nurse “ Special Needs Coordinator

1.0 QUALIFICATIONS

o Education: BSN Degree from an Accredited Nursing School

o Experience: 12 months in Nursing. In addition, experience in Family Health Nursing Certifications:

o ACLS, BLS,

o License: Current, full, active and unrestricted license to practice as a RN in any state

o Registration: N/A

o Security Clearance: Must have a clear background in order to pass the security clearance

o Familiar with organization structure and operation methods of Air Force

o Possess excellent written and oral communications skills. Effectively use correct grammar and spelling in all written communications.

2.0 DUTIES

Ensures patient safety and adheres to infection control IAW established guidelines.

Adheres to and enforces emergency, fire, and ground/facility safety policies and procedures. Oversees the planning, coordinating, and evaluation of nursing care delivery in an outpatient clinic. Nursing assessment and treatment may be conducted via in-clinic, telephone, and/or electronic contact in pediatrics.

Provides comprehensive professional nursing care to normal and high risk patients and documents actions.

Performs patient check-in, triage, telehealth, screening, and walk-in care for a variety of treatments and diagnostic tasks.

Conducts individualized patient/family counseling or education. Renders appropriate nursing care utilizing applicable Clinical Support Staff Protocols (CSSP).

Participates in disease and case management for the defined population.

Executes medical orders, appropriately delegating responsibility for implementation to qualified personnel.

Performs procedures such as eye and ear irrigation, nebulizer treatments, catheterizations, EKGs, visual acuity tests, venipuncture, specimen collection and processing, etc. within scope of practice and IAW established ambulatory nursing procedures.

Administers medications IAW ambulatory nursing guidelines, including narcotics and prescribed intravenous therapy.

Utilizes applicable information management systems in daily practice for patient care and administrative duties.

Facilitates collaboration with all levels of health care management to include outside agencies. Familiarizes with clinic and medical group operating instructions.

Develops tools and monitors metrics to evaluate and continuously improve the effectiveness of process improvement initiatives.

Facilitates evidenced-based clinical practice.

Coordinate patient care in collaboration with a wide array of healthcare professionals. Facilitate the achievement of optimal outcomes in relation to clinical care, quality and cost effectiveness.

Ensure compliance with standards of care and practice in accordance with all established policies, procedures, and guidelines used in the MTF.

Perform physical exam and health histories.

Provide health promotions, counseling, and education.

Administer medications, wound care, and numerous other personalized interventions.

Direct and supervise care provide by other healthcare professionals.

Accountable for making patient care assignments based on the scope of practice and skill level of assigned personnel.

Recognize adverse signs and symptoms and quickly react in emergency situations.

Communicate and collaborate with a diverse group of people for the purpose of informing the healthcare team of plans/actions, for teaching/education to benefit the patient/family and organization.

Make referral appointments and arrange specialty care as appropriate.

Conduct research in support of improved practice and patient outcomes.

3.0 WORK HOURS

Can range between 0700 and 1700, Monday through Friday, with a one-hour unpaid lunch break per day. The number of hours will not exceed 40 hours per week.
