Fortune Top 50 Company.We currentlyhave one opening for this position which may be located in any Anthem State.Remote is available, however, incumbent must be in a reasonable commutingdistance from a primary office location. Also must be available to work local hours.Responsible

for providing exercise guidance to disease management members.Primary

duties may include, but are not limited to:

Collaborateswith nurse case managers in managing disease management health goals ofmembers.

Implementsthe primary care physicians exercise recommendations providing safe andeffective exercise instruction and guidance.

Providesmember health coaching, education, assessment and support of behavioral changesto promote self-management of chronic conditions.

Providesmembers with education materials related specifically to member scorecondition.

Requiresa BS in exercise science, exercise physiology, kinesiology, physical therapy orphysical education;

3years of experience as an exercise specialist, exercise physiologist or fitnessinstructor/trainer developing exercise prescriptions for special populations;or any combination of education and experience, which would provide anequivalent background.

1year of cardiac rehab, pulmonary rehab, and physical therapy experience inclinical setting preferred.

Certificationby the American College of Sports Medicine (ACSM) as a Health FitnessInstructor preferred.

MSin a related field and membership in American Society of Exercise Physiologistpreferred.

Strong communication skills

Proficient in Microsoft office products
