- Ability to communicate, mediate and negotiate complex issues with a high level of organizational skill.
- Respects and maintains confidentiality of patient information obtained relative to case management activities.
- Work independently with minimal direction, seeking consultation and guidance as appropriate.
- Functions as case management liaisons with customers of the health care system, both external and internal.
- Operates office equipment including but not limited to telephone, computer keyboard with access to management information systems and tools, order communications printer and fax machine.
- Effectively communicates via written, verbal, face-to-face, telephone and computer methods.
- Recognizes that variables such as age, race, nationality, culture, disease-causing vulnerabilities, and socioeconomic factors affect the patient populations under their care as defined in the unit's provision of care statements.
- Uses interventions appropriate to these factors (e.g., interpreter, dietary preferences, social service consult, etc.)
- Minimum six years of clinical experience within acute care setting
- Must Be RN
- Bachelor Degree in Nursing
