American Medical Home Health Services is hiring in Del Rio, Eagle Pass and it's surrounding communities. We are searching for Full Time/PRN Registered Nurse's and LVN's to join our staff and deliver quality home care services.
Key responsibilities include:
RN:
Performs clinical assessments to identify patient needs and appropriateness of patients as evidenced by documentation, clinical record, case conference, team report, and evaluations.
Documents, prepares, revises as necessary, and completes a Plan of Care, patient notes, phone orders, and other clinical records documentation on an ongoing basis.
Supervise and provide clinical direction to the HHA/LVN to ensure quality and continuity of services provided.
Monitor assigned cases to ensure compliance with requirements of third party payor.
Uses effective interpersonal relations and communication skill.
Responsible for timely completion and synchronization of OASIS visits within patient software (Kinnser).
Education/Experience:
Graduate of an accredited school of professional nursing.
Minimum of 1 year of nursing experience in a home health setting.
Home Health and OASIS experience required.
Current License as an RN in the state of Texas.
LVN:
LVN's provide skilled nursing care to home care patients according to a Plan of Care (POC) approved by the physician under the supervision of the Registered Nurse (RN). This position is on a PRN (as needed) on a per visit basis.
Provide health education to patient and or caregiver.
Follows up with physician, client, family members, staff, and RN on client problems and concerns. Maintains communication with physician including report of client condition change and accepting verbal orders.
Completes accurate and timely documentation of care provided.
Be a self starter with excellent communication skills, and a team spirit.
AMHHS seeks the following qualifications:
Education/Experience:
Be a Licensed Vocational Nurse from an approved school of vocational nursing.
Minimum of 1 year of HOME HEALTH experience.
Current License as an LVN in the State of Texas.
For Both RN/LVN:
Knowledge of Wellsky(Kinnser) software required.
Travel is required; must have a reliable car w/current insurance coverage and a current driver's license.
English/Spanish speaking a plus.
Current TB Test.
Current CPR certification.
