I) Job Purpose

The service coordination mentor position will assist units in maintaining updated training information, coordinating and documenting unit specific training for new employees, and task specific training to Service Coordinators.

Maintain current training records to include monthly training rosters for all current and new employees. The work requires initiative and independent judgment.

The work requires aggressive intervention in the community as well as interaction with other internal departments within MHMR.

The work also requires completion of required documentation.

One of the goals of Disability Services is to provide the people we serve with seamless consumer driven services and supports.

Towards this end, the Mentor is expected to recognize his / her responsibility to collaborate with the Service Coordinators, to communicate within and across programs, services and supports and to be knowledgeable about the array of services and supports available in the community.

II) Essential Functions

The Mentor will train DS Service Coordinators new to the service coordination unit specific protocol and procedures. The mentor will provide ongoing training to Service Coordination units as well as other identified personnel.

The Mentor will train and assist with the development and monitoring process of Person Directed Plans (PDPs), Individual Service Plan (ISP) and assist with the development of the initial, revision, and renewal Individual Plans of Care (IPC);

crisis intervention activities; service planning and coordination; and monitoring. The Mentor will maintain current and up to date knowledge on DADS rules and regulations as well as MHMR policies and procedures.

A. Mentor, train, and assist the Service Coordination units in facilitating the Person directed Planning (PDP) process; Mentor Service coordinators in information gathering, the facilitation of the PDP meeting and in the overall development of the Person Directed Plan.

Performance Standards

Assist Service Coordinators in the information gathering process- assure that pertinent historical information (e.g., Nursing assessments, summaries / reports by specialized therapies, etc.

is reviewed and incorporated in the PDP or ISP.

Assist Service Coordinators in facilitating the Person Directed Planning (PDP). Individual Service Planning (ISP) process.

Mentor positive and cooperative interactions with MHMRTC personnel as well as Private Providers to obtain pertinent information about the individual being served.

Assist in developing narrative of results of PDP / ISP, as requested; assuring all required components are present.

Assure that all processes and service planning respects the needs, assessments, requests and preferences of the person.

These processes should also be directed towards the achievement of the person's goals and desired outcomes.

Assure that anytime barriers to the person's personal goals and desired outcomes are discovered, they are addressed in a timely manner.

Coordinate to assure strategies are developed to meet outcomes in the PDP / ISP for each individual served.

B. Mentor, train and assist the Service Coordination units in the monitoring process of the Person Directed Plan (PDP) Individual Service Planning (ISP).

This includes, but is not limited to, assistance with the monthly monitoring of services, PDP / ISP Update process, and identification of critical need.

Performance Standards

Assist Service Coordinators in the monthly review process. Mentor comprehensive record reviews, consultation with Provider personnel and consultation with the consumer and / or his / her family / LAR.

Assist in assessing implementation of services and supports, assuring that services are being provided to meet the needs, requests and preferences of the individuals receiving services.

Assist with the development of PDP / ISP Updates to incorporate any changes. If not originally covered in the PDP / ISP, assure that the consumer's most current needs, assessments, requests and preferences are reflected in PDP / ISP Updates.

C. Provide crisis intervention, which may include emergency access to service coordination.

Performance Standards

Properly notify program Manager / director and or designee of emergency situations.

Document all emergency situations within 24 hours according to organizational policies.

D. Meet performance timelines and participates in Divisional, Departmental and Agency Meetings.

Performance Standards

Completes and submits travel vouchers / reimbursement forms according to agency procedures 100% of the time.

Maintains a high attendance record and notifies Program Supervisor of intentions to be absent by start of workday.

Completes and submits SALs as needed and necessary.

Completes all required annual refresher training 100% of the time.

Attends all departmental meetings on a regular basis and meetings requested by your Program Manager.

Exhibits strong customer service skills by :

Receiving a limited number of verified consumer complaints (less than 4 annually) related to communication or actions.

Being focused on providing quality and timely services and information to all consumers served in a consumer friendly manner.

Working with fellow employees as well as community contacts in a professional manner.

Complete all organizational required training within first 60 days of hire.

Attend seminars, workshops, etc. to maintain up to date information regarding all issues related to service coordination.

E. Additional Responsibilities

Performance Standards

With the understanding that this position at times requires time sensitive responses, it is important for Mentor to be responsive and communicate in a timely manner with their supervisor.

Adhere to a typical work schedule; any changes are to be approved by supervisor ahead of time.

Utilize outlook to manage appointments and training; Outlook must remain up to date.

Additional training materials must be approved by Management Team prior to implementation so can maintain comprehensive training Manuals.

Represent the unit and / or MHMR when involved in program planning policy development, intra agency, and / or interagency coordination of Medicaid service programs.

Participate in regularly scheduled meetings to develop strategies to increase Medicaid system capacity, focus Medicaid programs on specific populations and other related goals Exercise decision making authority as representing the units and / or MHMR when involved in programming planning, policy development, intra-agency and / or interagency coordination of Medicaid service programs.

Pilot new tools for planning and supports with people served to determine viability for the entire unit.

Perform other duties as assigned.

V) Travel

This position requires periodic local travel with occasional out-of-area travel for trainings and meetings. This may involve use of the agency vehicle or use of personal car with mileage reimbursement.

This position may require temporarily or permanent re-assignment to any of the DS facilities as determined by the program needs.
