Physical Therapist Assistant - PRNLocation: LUFKIN, TEXAS, UNITED STATESNew Requisition #: 2019-R0268544Physical Therapist Assistant - PRN Position Summary: Under the direction of Director of Rehabilitation Services and Physical Therapist, the Physical Therapist Assistant (PTA) assists in providing physical therapy services including but not limited to screening, educating, and providing therapeutic intervention for patients identified with disabilities that interfere with their ability to perform daily life activities. Assist PT in development of individualized programs and exercises judgment and decision making within the limitations and scope of practice.Displays professional, safe conduct in all interactions. Maintains positive working relationships with staff, other departments, physicians and peers. Displays conduct in support of CHI St. Luke’s Health Memorial Mission & Vision.Position Responsibilities:

Plans and implements physical therapytreatment programs according to physician referral, and/or plan of care.Designs programs to meet the patient’s physical, psychological, and age specificneeds. Performs tests, and measurements of functional status, neuromuscular,musculoskeletal, cognitive/perceptual, and sensory /psychological functions.

Works collaboratively with all members ofthe interdisciplinary team, other departments and disciplines to assurepatients and customer’s needs are met in a timely manner; encourages the activeparticipation of the patient and/or family in goal setting and patient/familytraining/education.

Completes all necessary documentation asoutlined by departmental policy and documentation review. This includes, but isnot limited to, patient care charting, charge entry, completing peer chart andpeer reviews, and documenting on daily schedule.

Coordinates care of patients with otherdepartmental and hospital activities in a manner that fosters a positiveprofessional image consistent with the departmental and hospital goals andmission statement. This includes, but is not limited to, discharge planning,participation in care conferences, interdisciplinary rounds, collaborativepractice groups, the student program and community endeavors.

Assigns, supervises and reviews the care ofpatients and duties of the therapy technician.

Promotes and supports positive andprofessional customer service, while interacting with clients, guests andhospital personnel. Works collaboratively with all members of theinterdisciplinary team, other departments and disciplines to assure patientsand customer’s needs are met in a timely manner. Encourages the activeparticipation of the patient and/or family in goal setting and patient/familytraining/education.

Provides positive and constructive feedbackto enhance departmental operations.

Utilizes available information managementsystems to support departmental and hospital goals. This includes utilizationof hospital wide e-mail, information announcements, policies, education, chargeentry, and accessing patient care related information. Access and use ofinformation will meet hospital and information management guidelines.

Demonstrates tact and professionalism in workingand communicating with patients/families, physicians, other members of therehabilitation team, and other employees of the healthcare system.

As a member of the Rehabilitation Services Team,ensures that the patient/family is satisfied with the rehabilitation servicesprovided any that any issue that arises is addressed and resolved promptly.

Complies with and develops efficient andcreative ways to maintain departmental productivity standards.

Adheres to CHI St. Luke’sHealth Memorial policies, including but not limited to “CHI Values & Ethicsat Work”, HIPPA, PHI, ePHI, Privacy Rule, and confidentiality.

Maintains safe work habits;adheres to safety rules; immediately reports any unsafe condition; immediatelyreports any incident / injury.

Supports department-based goals and contributes tothe success of the organization.Complies with hospitalpolicies and procedures; attends required in – services, education andtraining.

Performsduties in accordance with that of hospital policies and procedures, andregulatory agency guidelines.

Wears identification badgeand clocks in appropriately.

Demonstrates a willingnessto follow the chain of command, regularly channels suggestions, criticisms andcomplaints to the appropriate person.

Performs other duties as assigned.

Minimum Qualifications: Education Level Required: Associates Degree - Physical

TherapyLicensure/Certification Required: Current Texas State Licensure (PTA) (PTOT)- issued by Executive

Council of Physical Therapy and Occupational Therapy Examiners – PTA (ECPTOTE)BLS certification (through an approved AHA program).Preferred: Member of the American Physical Therapy Association (APTA) and the

Texas Physical Therapy Association (TPTA).Knowledge, Skills, andAbilities Must be able to effectively use computer and computer

programs for documentation, departmental information and inter/intra

departmental communication.Experience Required: NonePreferred:2-3 years of
