Redeem Center HCS is hiring an RN to provide HCS (Home and Community-Based Services) nursing coordination to individuals with intellectual disabilities in group home and natural home in the Hidalgo County area. Our program is unique and rewarding for anyone interested in being part of promoting independence for people with intellectual and physical disabilities.

Duties:
Case management of medical appointments, medication monitoring, review of records, request of medical records, attending IPC meetings (Individual Plan of Care), development of Nursing plans for each individual etc.
Nursing Assessments
Delegation of tasks
Trainings of direct staff/ Special Needs training
Over-the-phone triage
Other duties as assigned.
Qualifications
Knowledge of HCS Principals
Strong organizational skills
Effective written and oral communication
Computer proficient in Word and Excel
Excellent time management
Continued education
Team player
