Heart,Advocacy,Love, andOutreach!#HALOWe are looking for a full-time Skilled Visit Clinical Manager, with Registered Nurse (RN) credentials,with experience in an office-setting, health-care profession, or related field. Job duties include but are not limited to:
- Performs education, and ongoing evaluation of clients who are receiving services under a medically approved Plan of Care.
- Completes assessments in a timely manner and in accordance with Facility Policy and Procedure.
- Develops a written plan to provide information and direction to other health team members.
- Documents all findings, plans, interventions, and client progress in the clinic record.
- Communicates plans and changes to the physician and/or nursing supervisor and other facility staff through the Care Plan, written progress notes, and participation in care conference.
- Prepares social histories to augment existing services, or a guide in determining or changing level of service, taking into consideration.
- Family relationship
- Meaning of illness to the client and/or his/her family
- Cultural attitudes in relations to illness
- The financial impact of client’s illness upon the family
- Demonstrates team work and effective communications to accomplish client, team and facility goals.
- Participates in case conferences and other team or facility meetings.
- Educates team members and clients/families about community resources and how to assess them appropriately.
- Performs other related duties and responsibilities as deemed necessary.
Please apply if you have the following qualifications.
- Current Registered Nurse (RN) in good standing and currently licensed by the State of Texas.
- Graduate of an accredited school of nursing.
- Valid driver’s license and auto insurance.
- Home Health, pediatric, and/or office experience a plus!
