Promotes excellence in patient care by advancing the quality and scope of nursing practice through clinical practice, education, consultation, research and administrative roles in the area of clinical expertise. Nursing activities include, but are not limited to, systematic analysis of health data, care plan development, health education and advocacy, coalition building, and collaborating with community partners to promote the health of a population.This position is funded through September 2021, and is potentially and likely renewable for additional years based on funding availability.This position is considered Category I: Mandatory and required on campus or health care facilities to support on-site operations. Employees hired into this category will be required to be physically located on campus if they are healthy and without COVID -19 symptoms.ResponsibilitiesOversees the care of patients treated in the facility to ensure orders are interpreted and properly executed by clinical personnel.Manages and coordinates nursing care, including providing patient careProvides care to patients with complex problems exhibiting creativity and innovation in nursing intervention.Works intra- and inter-departmentally to develop and evaluate standards, policies and procedures for provision of a safe and therapeutic patient environment.Provides and/or coordinates patient/family education and planning. Develops and evaluates education programs and materials to facilitate patient education and care coordination.Provides clinical education to patients, family and staff. Develops and conducts education programs utilizing internal and external resources based on an evaluation of continuing education needs.May function as a liaison between patients, family members, providers, and staff.Functions as preceptor for undergraduate students.Functions as consultant to staff in complex patient care situations. Facilitates and/or conducts research independently and/or collaboratively.Evaluates care provided to individual patients and/or groups of patients based on specialty area.Responds to Covid-19 pandemic through testing, follow up and contact tracingProblem SolvingThe primary area for problem solving for this position relates to management of the patients’ clinical care. The public health nurse provides care coordination, referral and follow-up to individuals and families who are members of a vulnerable population and/or high-risk groups. The nurse evaluates outcomes, effectiveness of plan, and makes changes as necessary. The public health nurse uses public health surveillance/disease investigation methods in community outreach, screening, and case finding of communicable and infectious diseases that threaten the health of the community. The nurse will advise organization leadership on the most effective methods of improving population health and assist in the setup and training staff in these services. This includes making decisions about referrals to dietitians, social workers, and especially physicians to ensure medical problems and/or symptoms are dealt with appropriately. This may also include arranging for medical procedures such as laboratory studies, EKG’s or other medical follow-up. Secondly, it is important that appropriate documentation identifying both the problem, and the resolution of that problem through clinical needs, has been completed.The public health nurse makes decisions independently and in collaboration with the clinic staff and leadership. Public health nurse is placed in a decision making position based on expertise, personal and/or delegated authority. The public health nurse interacts with members of the nursing Practice Department, physicians, peer groups, other disciplines, State of Utah board of Nursing, members of specialty nursing organizations, faculty at University of Utah and other local and national colleges and universities, members and leaders of the community. The scope of the Public Health nurse’s authority is to make nursing practice decisions in his/her area of specialty is broadly defined by Urban Indian Center Clinic Nursing Practice and disease specific policies and procedures. Major challenges encountered by the incumbent include problems related to patients, patient’s families, health policy, ethical dilemmas and staff conflict management.CommentsWork Environment and Level of Frequency that may be required:Nearly Continuously: Office environment.Often: Infectious disease, bodily fluids.Physical Requirements and Level of Frequency that may be required:Nearly Continuously: Hearing, listening, talking, walking.Often: Repetitive hand motion (such as typing), standing.Seldom: Bending, reaching overhead.Minimum QualificationsThis position requires a Bachelor’s degree in Nursing or equivalency; current R.N. licensure in the State of Utah; two years of professional experience in a clinically related area; a Basic Life Support health care provider card; and demonstrated team leadership, relationship building, critical analysis, and written and verbal communication skills. Knowledge of funding resources and clinical standards and outcomes is also required. A Certified Case Management designation is preferred. Previous experience in clinical resource management activities and third party payer interactions is preferred.This position is patient-sensitive and must fulfill all associated requirements.
