Direct Support Provider (DSP) (Forest)

OBJECTIVE OF POSITION: To provide direct support services to individuals in JMG Home Health Services Inc. and community according to directions and scheduling as specified by the individual, the Individual Service Plan, JMG Home Health Services Inc.

DUTIES OF POSITION:

Read and follow the service plan for the individual who receives support.

Protection of the health and safety of the individual is the primary focus for the service.

Directly provide congregate residential services to the individual in the home or in the community settings including completion of training for short term objectives listed in the individual's service plan.

The emphasis of work for the hone placement will be on self-care, basic domestic skills, communication/social skill

development, community integration, music, and art expression.

Complete documentation for program implementation, medication administration and to report on activities and unusual conditions or events.

Recognize emergency medical conditions and report such by calling 911, and then reporting natural family/AR, and JMG Home Health Services Inc. /Director as specified by the contact information located on the Emergency Medical Form for the individual.

Report unusual conditions or emergencies to appropriate authorities and to JMG Home Health Services Inc. according to the Human Rights Plan and Individualized Service Plan.

Maintain confidentiality as required by the Health Insurance Portability and Accountability Act.

Demonstrate a commitment to ethical service provision-using the code of Ethics for Direct Support Professionals (DSP)as a guide.

All service providers are "mandated reporters" and must report all suspected individual abuse and neglect within 48 hours to the local Department of Social Services, Adult Protective Services, Human Right regional advocate and/or JMG Home Health Services Inc. Director.

This position will have a minimum of 20 hours per week with the possibility of additional hours as needed. Typical week will be M-F 4-9 pm.

KNOWLEDGE, SKILLS AND ABILITIES (KSA) expected of EMPLOYEE

Knowledge of:

Principles and philosophy of community involvement for individuals with developmental disabilities

Concepts of positive behavioral supports

Specialized and general knowledge of the implementation of individualized training programs Documentation and data collection systems Licensure regulation including human rights policy and other standards and laws First Aid, CPR, infection control and medication administration Human rights policy

Emergency response procedures

Code of Ethics for Direct Support Professionals

Skilled at:

Oral and written communication

Interpersonal skills particularly in terms of building trust and rapport with persons who have special needs Observing and documenting behavior Responding to the special needs of people with disabilities

Implementation of training programs for special needs population

Ability to:

Use common sense approaches to implement services

Carry out training and behavior support programs

Appropriate implement professional confidentiality practices

Demonstrate a commitment to quality, community based services for people with disabilities

Communicate to other the nature and philosophy of the services offered

OTHER REQUIREMENTS

A criminal history check with results showing no indication of a criminal history that would be a barrier to employment as determined by DBHDS

A Child Protective Services form with results showing that this person had NOT been identified in the Central Registry ChildAbuse/Neglect

A DMV driving record which indicates a safe driving history (if transporting individuals)

A TB test Behavioral Management Training (such as TOVA MANDT), etc.

Completion of orientation manual test for Developmental Disabilities Waiver

A High School diploma or equivalent 1 year of documented experience working with DD populations
