SUMMARY

Will provide intensive care coordination services to youth and families in the community.

ESSENTIAL DUTIES AND RESPONSIBILITIES

Maintain a caseload of up to 10 ICC clients and families. Working with youth and families to create teams, develop goals, plan and coordinate transitions, and monitor progress to ensure that a youth can maintain in the community with their family.

Implement the 10 principles of High Fidelity Wraparound.

Conduct an initial and discharge assessments

Identify immediate and/or potential crises; develop and ensure implementation of safety plan as needed.

Engage, support, and facilitate conversations with family, youth and team members regarding service planning and implementation.

Assess the placement and treatment needs of clients in Treatment Foster Care program to meet their service plan upon intake into the program.

Conduct home visits to assigned clients to assess client’s level of care, and provide guidance toward meeting their treatment goals.

Provide on-going support to resource parents, guiding and encouraging them as they care for clients in their home.

Provide service coordination among referring agencies such as the Department of Social Services, Community Services Boards, Family Assessment and Planning Teams, and others.

Other duties as assigned

EDUCATION/EXPERIENCE:

Bachelor’s degree (BSW) in social work or related degree with two years direct service experience. MSW or related preferred.

Demonstrated ability to apply sound clinical casework and counseling skills to children and families, understand treatment modalities and their implications for services to families and youth.

Completion of “High Fidelity Wraparound” training preferred.

Ability to work a flexible schedule, including nights and weekends, in order to meet the needs of ICC children and families.

Effectively use a variety of computer software applications in word processing, spreadsheets, and presentation software. Must be proficient in MS word, Microsoft outlook and use of the Internet to produce reports and analyze data. Must be able to learn and gain proficiency in program specific technologies such as Harmony.

CERTIFICATES, LICENSES, REGISTRATIONS, TRAININGS

Possess valid driver’s license with acceptable driving record and proof of automobile insurance coverage. Certification in High Fidelity Wraparound required.
