SE WA Aging and Long Term Care - $30 per hour
We are recruiting a PT Extra Help (Up to 650 hrs/year) Health Home Outreach Project Worker in our Walla Walla office. As a Health Home Care Coordinator, they will provide outreach and backup care coordination.
Experience and Training: Bachelor's degree in Social Work, Social Science, Gerontology, Psychology, Counseling, or related fields AND two years of direct service case management or social service program experience required (may be paid or volunteer experience within a social service agency). Experience meeting and working with people in homes and other medical and community settings. Experience in Motivational Interviewing or other empowerment-based approaches is desired. Bilingual Spanish Preferred. Closes at 5:00 pm, March 20, 2019
