Description:Swedish Medical Center is looking for a Clinical Social Worker $5,000 Sign-On Bonus (1.0 FTE, Day Shift) to work in Case Management - Fh at Swedish First Hill in Seattle, WA.The Clinical Social Worker partners with patients, families and the health care team to address and advocate for patients' social and emotional needs. The clinical social worker is responsible for providing a full range of social work services including but not limited to psychosocial assessment, treatment planning, therapeutic interventions, discharge planning, crisis intervention, and resource referral. The clinical social worker partners with the patient and his or her support system, as well as interdisciplinary teams, both internal and external to the organization, to improve patient care through the effective utilization and monitoring of health care resources. The clinical social worker assumes a leadership role to facilitate the achievement of patient goals and desired clinical, financial, and resource outcomes. The clinical social worker is guided by the NASW Code of Ethics and the Swedish Mission and Vision in his or her daily practice.In this position you will:Collaboration: Works collaboratively with patients nurses, case managers, physicians, other practitioners, caregivers and community resources and agencies. The social worker and healthcare team are jointly accountable for measurable outcomes that are cost effective and reflect patient preferences and values. Contributes to the development of a goal-directed, age-appropriate plan of care through an interdisciplinary team process that is prioritized and based on medical diagnosis, patient needs, and expected patient outcomes. Interacts with patients and physicians to explore the most appropriate setting to meet patient needs. Participates in the development, implementation, evaluation, and ongoing revision of initiatives to improve quality, continuity, and cost-effectiveness. Works collaboratively with other departments and services to define and study areas of inefficiency and participates in process improvement projects. Fosters positive internal and external customer relations. Collaborates with community providers and provides patient/family with appropriate community resources to promote achievement of agreed upon goals. Conducts substance abuse assessment, assesses patient’s readiness to change, provides information about treatment options, and refers or facilitates transfer of acutely intoxicated patients to the sobering and medical detox facilities. Conducts psychiatric assessment and intervention and refers patients to the appropriate level of care, including outpatient, voluntary inpatient, and involuntary treatment.

Communication: Communicates timely, relevant and accurate information to all parties involved with a patient’s care. Communicates patient needs related to advancing the medical treatment plan and/or discharge plan to appropriate professionals and follows up. Communicates with patients and families to ensure understanding of third-party payer guidelines and to arrange referrals. Provides clear and thorough documentation based on established department standards

Facilitation: Facilitates the progression of care by advancing the care plan to achieve desired outcomes. Develops discharge plan through collaboration with the interdisciplinary team. Ensures that all activities to facilitate and coordinate the plan are being implemented and that the plan is continuously modified based on the patient’s changing needs. Assesses for financial barriers to advancing the treatment plan and discharge plan, provides information about resources, and advocates for patient/family in accessing financial assistance programs. Assesses for and manages complex patient and family dynamics that impede the treatment plan and/or discharge plan to support and facilitate alignment of goals between the patient, family and healthcare team, including facilitating and participating in care conferences.

Qualifications:Required qualifications for this position include:Master's Degree in Social Work from an accredited school

Licensed Social Work Associate – Independent Clinical (LSWAIC) required.

Licensure as an Independent Clinical Social Worker (LICSW) with the Washington State Department of Health under Chapter 18.225 RCW is preferred and may be substituted

One (1) year experience in the areas of acute medical or mental healthcare in an inpatient or outpatient setting

Preferred qualifications for this position include:ACM/CCM certification
