Island Senior Resources is now hiring for a part-time (25 hour) Case Manager. Potential growth to full-time in 3-6 months.
Seeking an enthusiastic social services professional to join our Aging & Disability Resources (ADR) team. Work for an established non-profit to promote independence, health, and dignity for older adults in Island County.
This is a newly created position to provide support, guidance, and case management to high-needs individuals. Position will have a limited caseload to allow for 1:1 assistance to individuals who have multiple needs or are discharging from a hospital or facility who do not have adequate support and cannot navigate services on their own. This is part of our commitment to support individuals with care transitions and to reduce preventable hospitalizations.

This position will be located at the South Whidbey Center (in Bayview). Candidate may have some flexibility in establishing a schedule. Must be very independent and reliable.
