Ten-Step Hourly Range: $23.50 - $30.66, DOQ

We are seeking an energetic, enthusiastic and dedicated individual to join our Care Coordinator Team. Northwest Regional Councils Health Home Care Coordinators help clients establish health action plans and provide ongoing support to achieve established health goals.
Essential elements of the Care Coordination Program include:
ï Comprehensive Case Management
ï Care Coordination
ï Health Promotion
ï Transitional Care to and from Settings
ï Referral to Community and Social Support Services
The Health Home Care Coordinator will engage clients in establishing health action plans and provide ongoing support in achieving health goals.
