DescriptionThe Licensed Mental Health Counselor will function as a core member of a multidisciplinary team, work align side primary care physicians providing whole person health to individuals living with behavioral health illness. Provides bio/psycho/social assessment and behavioral health diagnosis to assure appropriate level of treatment referral. Supports patient in learning evidence based therapy skills to improve current symptoms. Acts as an advocate and helps patient gain access to resources while confronting their personal issues, such as mental illness, addiction, and abuse. The position takes responsibility for management of presenting behavioral health conditions and social determinants of health at time of primary care visit. Position treats individual and family care to children, adolescents, and adults as requested. Master's Degree in Mental Counseling or Behavioral Science

Bachelor's Degree in Behavioral Science
