Description:Providence is calling a Safe Crossing Counselor (Hospice, Day Shift, 1.0 FTE) to Providence Hospice of Seattle in Tukwila, Washington.Hospice caregivers will tell you what a privilege it is to care for patients and their families at the end of their lives. Provide individualized psychosocial assessment of child/family needs.
Provide consultation and assessment of needs to local school staff members. Implement service based on assessment and reflective of school staff and student goals.
Provide consultation for individual PHOS clinical staff to optimize support for families with children.
Develop and implement counseling interventions based on a plan of care, established policies and procedures, and reflective of child/family goals.
Teach children and parents regarding anticipated changes and related issues in grief/loss.
Schedule visits for optimal patient care, care management and team effectiveness.
Facilitate children’s grief groups in various settings including, but not limited to, agency and school.
Qualifications:Required qualifications for this position include:
Master's Degree in counseling, psychology or social work
Current Social Work Certification or License through Washington State Department of Health
Current Healthcare Provider Cardiopulmonary Resuscitation (CPR) certification
Current Washington State Driver's license and automobile insurance
3 years post master's work experience. Prior internship at PHOS or other relevant social services-related role counts towards the 3 years of post master’s work
1 year prior experience providing grief counseling to children and families
1 year experience in health care setting
Preferred qualifications for this position include:
2 years experience in health care setting
