Primary duties may include but are not limited to:

Directs the development, planning, prioritization, implementation and compliance of Care Delivery Transformation (CDT) program fundamentals.

Supports the development and execution of the Enhanced Personal Health Care CDT priorities within respective markets.

Manage external practice facing relationships and internal relationships with Provider Solutions market leadership, Medical Directors and Provider Clinical Liaison (PCL) leadership.

Ensures appropriate program tracking and reporting data for respective markets.

Works with CDT resources to develop and deploy tools, resources and documentation to support program delivery and the tracking/reporting of the program successes.

Provides primary oversight and direction of the Care Consultant Sr. in their assigned states.

Hires, trains, coaches, counsels and evaluates performance of direct reports.

Requires a BA/BS degree in health related profession (nursing, behavioral health, social work, dietetics) and/or Business Administration & Management, Healthcare Administration and Management or related field; 10 + years of experience in the healthcare and provider setting; managing staff in external client or practice facing program management, project management; complex business processes, strategic and business planning, data analysis; or any combination of education and experience, which would provide an equivalent background. MBA preferred.
