Summary

Hiring two Registered Nurses for the Cody, Park County Outpatient Clinic, demonstrate knowledge & skill in professional independent manner which will include: serve in variety of leadership roles; development & implement collaborative strategies with others to bring positive outcomes; providing good customer service; follow policy development & implantation; compliance with practice standards & regulatory standards; while providing quality of care and patient safety.

Responsibilities

The Registered Nurse (RN) Duties:

Responsibilities will fall under Primary Care, and Telehealth including occasional traveling to Veterans Homes

  • Demonstrates clinical knowledge in assessing, planning, implementing documenting and evaluating care for a designated group of patients across the continuum of care
  • Triage and apply a collaborative team approach in identifying, analyzing and resolving patient care problems
  • Collaborates with patients to assess and identify needs, issues, care goals and resources in critical factors for achieving desired outcomes for discharge, post hospital recovery and health maintenance/wellness
  • Maintains current knowledge of multidisciplinary resources, programs and services, referring patients for community resources as appropriate
  • Utilizes problem-solving methodologies and advances clinical knowledge to resolve system and/or processes that impact patient care
  • Assists with navigation of health systems and intervenes or refers as needed to facilitate the achievement of patient outcomes
  • Provides care, respecting patient's personal values, cultural and belief systems
  • Assists with the development, assessment, and monitoring/ evaluation of comprehensive plans of care to ensure efficient, effective, and coordinated care in meeting and promoting standards of wellness
  • Coordinates an interdisciplinary approach to assure physical, psycho-social, and environmental needs and resource delivery to achieve patient goals
  • Interfaces with a panel of rural or highly rural Veteran patients via computer to monitor daily responses from Veterans' home monitoring input devices
  • Completes assessment of patients who may have multiple and complex comorbidities that necessitate additional coordination of care and case management to ensure easily accessible, continuous, high quality care across all settings
  • Completes assessment and plan of care on assigned patients and provides individualized nursing care
  • Conducts ongoing reassessments to evaluate change of patients' status
  • Plans for discharge/transfer of patients including completing discharge/transfer notes in CPRS
  • Monitors recovery process and related medical, physical, and psychological problems and other interventions defined by unit policy
  • Promotes and provides health care teaching to patients, includes family members when feasible and appropriate in patient treatment
  • Manages the daily routine operations related to patient care and workload
  • Continuously improve work processes and offer suggestions for the improvement of patient care

Pay: Competitive salary, regular salary increases

Paid Time Off: Annual leave at the rate of eight (8) hours per biweekly pay period; Sick leave at the rate of four (4) hours per biweekly pay period; 11 paid Federal holidays per year

    Qualifications

    To qualify for this position, applicants must meet all requirements by the closing date of this announcement.

    Basic Requirements:

    • United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
    • Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing.
    • Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.
    Preferred Experience:

    • Out patient Primary Care nursing experience within the last 3 years
    Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:

    • Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required.
    • Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience.
    • Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience.
    • Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 year's of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 year's of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required.
    • Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 year's of nursing practice/experience; OR a Doctoral degree and approximately 2-3 year's of nursing practice/experience.
