    Summary

    As the Off Tour Senior Social Worker the incumbent may be called upon to assist with Veterans and their families in the Urgent Care, PACT and on Inpatient Units when there is a need that may require social work intervention during Non-administrative tours when direct supervision is not available. The incumbent will work a non-administrative tour which may include some compressed schedule and weekend and evening work.

    Responsibilities

    SCI/D Program and Veterans Home of Wyoming Admission Coordination

    The Incumbent is responsible for completing the following duties/responsibilities:

    • Prepares program reports for VA Central Office, the VISN, and Medical Center leadership including but not limited to the SCI/D registry.

    • Duties include providing and/or arranging for the provision of appropriate rehabilitation services and devices in order to enhance an SCI/D Veteran's functioning level.
    • Other duties include identifying new cases of SCI/D providing professional counseling, meeting specific objectives established by the SCI team.
    • Arranging SCI/D annual evaluations as appropriate for Veterans with SCI/D and conducting educational programs relating to SCI/D and related resources.
    • Participates as part of the SCI interdisciplinary treatment team and contributes to the plan of care for SCI/D Veterans
    • Organizes community services such as Spinal Code Injury/Spinal Cord Dysfunction Services, as well as arranging skilled & non-skilled home health and home hospice care on behalf of beneficiaries, developing and coordinating procedures for use of these services. Develops working relationships and agreements with other organizations having responsibilities for the same patient population. Maintains a current and productive network of referral resources. Makes provision for referral services from other agencies, community resources (i.e., transition or halfway houses, shelters, etc.), and other Governmental agencies to resolve problems.
    • The incumbent may be required to travel in order to provide outreach, meet with stakeholder, meet with veterans or participate in training related to the position.
    Clinical Services Approximately

    • The incumbent receives and completes consults from interdisciplinary teams and other social workers on complex, difficult cases, using advanced practice skills and expertise. Incumbent also provides consultation to referral sources and is responsible for developing and maintaining professional standards of practice and educating others regarding such. Incumbent documents independently in the medical record utilizing paper and/or electronic formats, including psychosocial assessments, diagnostics, treatment, progress notes, follow up and referrals.

    • The incumbent completes psychosocial assessments; interviews veteran and family members and/or significant others to establish facts about the veteran's unique situation; identifies presenting problems and their causes, and the impact of such problems on the veteran's functioning and health as a part of a comprehensive psychosocial assessment; assesses high risk factors, acuity and need for services. The social worker interprets and explains VA treatment and benefit programs.
    • The incumbent develops an assessment of the veteran in collaboration with the interdisciplinary treatment teams, the veteran, family members and significant others, whenever possible. The goal of the assessment is to highlight the veteran's strengths, limitations, and internal/external supports and service needs in order to provide the best intervention/services possible given the situation. It is the responsibility of the social worker, working with the interdisciplinary treatment teams and other referral sources to develop a treatment/care plan with the veteran based on this assessment. Incumbent uses advanced clinical training, insight and experience to interpret data and to identify viable treatment options.

    • The social worker uses known available resources and the initial assessment of the veteran's likelihood to accept differing types of assistance to make initial and continuing decisions regarding use of VA and non-VA services and referrals.

    • The incumbent Senior Social Worker provides independent consultation and makes recommendations to the interdisciplinary team regarding the course of treatment. These decisions are based on the social worker's advanced practice skills, professional judgment and expertise regarding the veteran's self-care capability, the family support system, the veteran's health care needs and possible consequences.

    • The incumbent provides the patient, caregivers, and/or family members with ongoing supportive counseling. The purpose of such counseling is to deal with the psychosocial impact of coping with chronic/disabling illnesses, onset of a catastrophic illness and/or death and dying; need for non-institutional or institutional services as needed during the course of treatment. The incumbent assists patients and/or caregivers in coping and dealing with the loss and grief experienced in chronic illness, terminal illness and death.
    • The incumbent will participate in discharge planning with other disciplines and will be responsible for ensuring that discharge plans are executed in a manner that is timely and appropriate.

    Qualifications

    Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.

    Basic Requirements:

    • United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
    • Education. Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work.

    • Licensure. Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Current state requirements may be found by going to http://vaww.va.gov/OHRM/T38Hybrid/.

    • English Language Proficiency. Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. § 7403(f).

    Preferred Experience: Have 4 years post graduation experience.

    Grade Determinations:

    Social Worker (Program Coordinator), GS-12

    (1) Experience and Education. One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience.

    (2) Licensure/Certification. Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure.

    [(3) Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, the candidate must demonstrate all of the following KSAs:

    • (a) Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. (b) Knowledge and ability to write policies, procedures, and/or practice guidelines for the program.

    • (c) Ability to supervise multidisciplinary staff assigned to the program.

    • (d) Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program areas.
    • (e) Ability to provide training, orientation, and guidance within clinical practice.

    Physical Requirements: This position provides direct services to patients and their families through casework. Incumbents must be physically and mentally able to efficiently perform the essential functions of the position without hazard to themselves or others. Normal work of the position requires visual acuity (with the ability to read typewritten characters without strain, corrective lenses permitted), keen hearing, clear distinctive speech, and manual dexterity. This position requires potentially moderate levels of lifting and carrying (15-44 pounds). The incumbent may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs. Must complete annual Employee Health requirements, such as annual TB screening or testing, as a condition of employment. Must have a valid driver's license in a state. May be expected to operate Government Vehicle.
