This is an OPEN CONTINUOUS ANNOUNCEMENT and will remain open until August 11, 2023. The initial cut-off date for referral of eligible applications will be May 26, 2023. Eligible applications received after that date will be referred at regular intervals or as additional vacancies occur on an as-needed basis until positions are filled. Selected applicant is responsible for providing care coordination services (including but not limited to biopsychosocial assessment, walk-in/crisis services, education, provision of self-management resources) and other clinical tasks as needed for Veterans receiving services in BHIP. Care coordination (including care management and/or case management) includes determining and monitoring care coordination needs over time, providing care coordination, and facilitating follow-up and outreach as needed to promote proactive, personalized care, and care continuity. Position Responsibilities include, but are not limited to:

  • Care Coordination/Case Management serving as an advocate on behalf of the Veterans, and their families, assigned to a BHIP team to ensure delivery of comprehensive, seamless mental health services is maintained throughout the continuum of care. This includes communication within and across care lines to facilitate continuity of care for Veterans.
  • Act as a clinical resource/point-of-contact to Veterans and other team/staff members .
  • Conduct biopsychosocial assessments to determine care coordination and treatment needs of Veterans.
  • Provide consultation/education to Veterans and families on VA resources (including self-management tools), community resources, Advance Directives, and VA benefits.
  • Actively participate in the treatment planning process with other disciplines.
  • Participate in discharge planning with other disciplines and will be responsible for ensuring that discharge plans are executed in a manner that is timely and appropriate to promote care continuity.
  • Serve as liaison between VA and community resources, to include education and bidirectional communication that promotes seamless care.
  • Maintain familiarity and share responsibility with other team members for performance measures relevant to the BHIP team.
  • Serves on Interdisciplinary Treatment Team(s).
  • Provides crisis intervention when indicated including timely assessment of at-risk Veterans in crisis to identify immediate needs, evaluate risk, and initiate safety plan as appropriate.
  • Provide individual, family and group counseling as appropriate.
  • Provide wellness/prevention education and may facilitate patient/family educational/bridging supportive sessions as appropriate.
  • Document interventions and activities in patient's electronic health record and ensure appropriate hand-off communication and documentation when transferring patient care.
  • Actively participates in program planning and development, advising the supervisor of shifting trends with care coordination needs and caseloads and identifying the need for making recommendations on program procedures and/or policy changes based upon work experiences.
  • Takes responsibility for improving professional skills, participating in planning and executing specific staff development projects, attending seminars and institutes, and participating in training programs of other disciplines as requested.
  • Perform other BHIP-related duties as directed by the Supervisor, such as, but not limited to: Training/mentoring and oversight of new staff & those not yet clinically licensed and preparing for and training to staff on CARF & The Joint Commission standards and accreditation process.
  • Follow national guidance and local policy and SOPs for the MHSL. This includes following policies and documentation requirements as required for CARF and The Joint Commission accreditation as well as other internal and external requirements.
  • Participate in committee assignments, as deemed necessary by the service manager.

Qualifications

Basic Requirements:

-United States Citizenship

Be a citizen of the United States.

-English Language Proficiency

Must be proficient in spoken and written English.

-Education

Hold a master's or doctoral degree in: Clinical Mental Health Counseling; Clinical Rehabilitation Counseling; Clinical Mental Health Counseling and Clinical Rehabilitation Counseling; or a related field, from a program accredited by the Council on Accreditation of Counseling and Related Educational Programs (CACREP).

-Licensure

Persons hired or reassigned to LPMHC positions in VHA must hold a full, current, and unrestricted license to independently practice as a LPMHC, which includes diagnosis and treatment.

Exception

VHA may waive the licensure requirement for persons who are otherwise qualified, pending completion of state prerequisites for licensure. This exception only applies up to the full performance level. For grade levels at or above the full performance level, the candidate must be licensed. The appointing official may waive the requirement of licensure for a period not to exceed 3 years for a LPMHC that provides care under the clinical supervision of a licensed LPMHC who is at or above the full performance level. This

exception only applies at the entry level (GS-09).

Grade Determinations:

LPMHC, GS-09

-Experience, Education and Licensure

This entry grade level is for LPMHCs with less than one year of experience(post-master's degree) or for LPMHCs at the master's or doctoral level who are graduates not yet licensed at the independent practice level. Unlicensed LPMHCs at the GS-9 grade level have completed the required education listed above and are working toward completion of prerequisites for licensure.

Candidates must demonstrate the following:

  • Engage in intake and assessment, problem identification, diagnosis, treatment planning and implementation, continuum of care, evaluation and follow-up.
  • Knowledge of current Diagnostic and Statistical Manual (DSM) of Mental Disorders in formulation of treatment goals and application of appropriate clinical interventions using professional counseling practices.
  • Basic knowledge of professional counseling practice with emphasis on human development principles through cognitive, affective, behavioral or systemic interventions, and strategies that address high risk behaviors, wellness, personal growth, and career development across the life span.
  • Use a wide variety of individual, group, or family counseling interventions; demonstrates sensitivity to diversity and possesses multicultural counseling skills.
  • Serve on an interdisciplinary team with focus on the intrapersonal, interpersonal, and relational functioning of individuals, groups and families and on the counseling approaches, strategies, and techniques that will best help patients function effectively.
  • Basic skill in the use of computer software applications for drafting documents, data management, maintaining accurate, timely and thorough clinical documentation and delivery of services, such as Telehealth.
  • Learn and utilize software programs in use by VHA.
  • Knowledge and understanding of existing relevant statutes, case laws, ethical codes, and regulations affecting professional practice of counseling.
  • Communicate effectively, both orally and in writing, with people from varied backgrounds.

LPMHC, GS-11

-Experience, Education and Licensure

Candidates must have at least one year of post-master's or post- doctoral degree mental health counseling experience (VA or non-VA experience) and must be licensed to practice at the independent practice level. LPMHCs may provide clinical supervision over the clinical practice of an unlicensed LPMHC or LPMHC trainee. Some state licenses do not license an individual to provide clinical supervision to trainees or unlicensed LPMHCs.

  • Make professional evaluations, decisions, and recommendation for treatment planning and implementation.
  • Advanced knowledge and mastery of the fundamentals of the counseling process which includes defining patient/family problems and maintaining an effective counseling relationship.
  • Provide subject matter consultation to colleagues and trainees on the counseling process within various specialty areas, build on the foundation of competence through regular meetings and discussions to explain assignments, review progress of cases and confer about the counseling perspectives and orientation.
  • Provide complex crisis intervention and stabilization to patients who are in psychological distress. Requires independent judgment and skill.
  • Establish goals/treatment through a collaborative process with the patient utilizing advanced counseling skills, including evidenced- based practices, screening, and psychosocial assessment.
  • Use a wide variety of individual, group, or familial counseling interventions; demonstrates sensitivity to diversity and possesses multicultural counseling skills.
  • Utilize the current DSM in making diagnoses and formulation of treatment goals and application of appropriate clinical intervention using professional counseling practices.
  • Develop and facilitate psychotherapy and psycho- education groups that include life skills, family support, and community integration. This may include evidence based psychotherapy.

Preferred Experience: Position is responsible for case management, coordination, and continuity of care for Veterans receiving services throughout the Mental Health Service Line. They will use a high level of skill and creative problem-solving in assessing and treating the complicated biopsychosocial problems of Veterans who typically experience chronic and progressive medical and mental health conditions and impairments, in collaboration with members of the interdisciplinary treatment team and community partners. Familiarity in case management and care coordination also preferred.

The full performance level of this vacancy is GS-11. The actual grade at which an applicant may be selected for this vacancy is in the range of GS-09 to GS-11.

Physical Requirements: This position provides direct services to patients and their families through casework. Incumbents must be physically and mentally able to efficiently perform the essential functions of the position without hazard to themselves or others. Normal work of the position requires visual acuity (with the ability to read typewritten characters without strain, corrective lenses permitted), keen hearing (aid permitted), clear distinctive speech, and manual dexterity to include use of fingers, both hands, ability for rapid mental and muscular coordination simultaneously. Typically, the incumbent sits comfortably indoors, however there are potentially long periods of continued walking; standing; sitting, bending; carrying of light items such as papers, books, small parts; or driving an automobile; etc. This position requires potentially moderate levels of lifting and carrying (15-44 pounds). The incumbent may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs.
