Summary

The Community Care Clinical Coordinator is a member of the Business Office, Community Care team with responsibility to coordinate purchased care in the community for Veterans receiving care through the Sheridan VA Health Care System. Services may be provided in conjunction with Primary Care and involve the delivery of health care by community care providers. The Clinical Coordinator is responsible to perform duties ensuring quality patient care and appropriateness of services.

Responsibilities

The Community Care Clinical Coordinator is a member of the Business Office, Community Care team with responsibility to coordinate purchased care in the community for Veterans receiving care through the Sheridan VA Health Care System (SVAHCS) and in its catchment area. Services may be provided in conjunction with Primary Care and involve the delivery of health care by community care providers. The Clinical Coordinator is responsible to perform both clinical and administrative duties, ensuring quality patient care and appropriateness/timeliness of services.

S/he possesses advanced knowledge and understanding of community care programs, eligibility requirements, standards/regulations, delivery of care processes, clinical documentation requirements, current standards for evidence-based treatment, and compliance requirements. Possesses and demonstrates ability to work independently, has excellent customer service skills, is able to multi-task, and functions as a member of a team in a manner that empowers the team to collaborate and resolve issues. Is able to independently prioritize work, set deadlines, and resolve problems. Has advanced oral and written communication skills, advanced computer skills, and data analysis skills.

With delegated authority from the Chief of Staff, the Clinical Coordinator is responsible for the clinical review and approval of purchasing care in the community, assuring orders/authorizations are appropriate, complete, and current, in order to meet Veteran's needs. S/he collaborates with Veterans,

caregivers, VA providers, community providers, and other team members professionally and respectfully, is a point of contact for Veterans receiving community care services, and is responsible for resolving clinical and administrative concerns related to their care. Communicates regularly with

SVAHCS providers including physicians, physician assistants, advance practice nurses, clinical coordinators, case managers, social workers, and any others involved in the care of Veterans receiving purchased community care services.

Additional responsibilities may include assisting with statistical reports/documents for meetings or inquiries, and functioning as a resource for colleagues as needed. S/he will also provide backup coverage for the RN Transfer Coordinator, RN Traveling Veteran Coordination and Community Home Health Care Coordinator as needed.

The functional role of the Community Care Clinical Coordinator requires collaboration with community agencies and VA staff to address and respond to Veteran's individual health care needs along the illness and care continuum, and across multiple settings. Responsibilities include collaboration with

community agencies, discharge planners, and Patient Aligned Care Teams (PACT) to organize, coordinate, and authorize services for community care. Ensures appropriate care is provided in a timely and efficient manner to meet Veteran's individual health care needs. S/he is aware of available

program resources (financial, human, material, or informational) to facilitate safe, effective, and efficient care; maintains knowledge of current techniques, trends, and professional issues; appropriately utilizes resources to identify, analyze and resolve care problems; and makes sustained contributions to health care by sharing expertise within and/or outside the medical facility.

As a Case Manager, the Clinical Coordinator ensures services are appropriate for Veteran's needs; authorizations are appropriate, current, and complete according to the latest guidelines; and collaborates with Veterans, caregivers, community care agencies, and team members professionally

and respectfully. Functions as a point of contact for Veterans receiving care in the community and assists in the resolution of clinical or administrative concerns related to their care.

    Qualifications

    Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.

    Basic Requirements:

    • United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
    • English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English.
    • Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing.
    • Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.
    Preferred Experience:

    Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:

    • Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required.
    • Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience.
    • Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience.
    • Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 year's of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 year's of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required.
    • Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 year's of nursing practice/experience; OR a Doctoral degree and approximately 2-3 year's of nursing practice/experience.
    Reference: VA Regulations, specifically VA Handbook 5005, Part II, Appendix G-6 Nurse Qualification Standard. This can be found in the local Human Resources Office.

    Physical Requirements: The incumbent must be able to perform all activities of the position without restriction. This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity in order to provide safe and effective nursing oversight and care. This position requires potentially long periods of continued standing and/or sitting. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs.

    Annual leave shall accrue for full-time Nurses, and Nurse Anesthetists at the rate of 8 hours for each full biweekly pay period.
