    Summary

    The Respiratory Therapist - Polysomnographer serves as the primary therapist in the sleep lab, performing the technical testing and monitoring of sleep studies. Incumbent may also provide basic and advanced Respiratory services to adults and geriatric patients, as well as diagnostic cardiac, neurologic, vascular, and pulmonary testing at the Sheridan VAHCS in a backup role capacity. This position is supervised by the Associate Chief of Staff (ACOS) for Medicine Service.

    Responsibilities

    The Therapist will monitor and assist in maintaining Sleep Lab equipment and ordering daily supplies for the Sleep Lab. The Therapist will maintain ongoing knowledge on current trends and techniques in the field of sleep diagnostics testing. Appropriately handles scoring, data transfers, report preparation, and document distribution in an efficient/timely manner. The Therapist may perform routine respiratory care procedures such as nebulizer therapy with specific bronchodilation drugs, oxygen therapy, humidification therapy, chest physiotherapy, postural drainage and sputum induction. The therapist will take initiative in reporting changes, complications, progress in each patient, and make recommendations for further treatment to physicians and members of the medical team and receive verbal/telephone orders from physicians to enter consults into CPRS for orders related to respiratory therapy or pulmonary function testing. The Therapist will review, collect and evaluate relevant clinical data from the medical records. The Therapist will be capable of performing both basic and complex therapeutic procedures for adults and geriatric patients. Example: monitoring patients receiving mechanical ventilation, assisting with bronchoscopy, using techniques that include interpretation of clinical data, describing and reporting arterial blood values and acid base balance. The Therapist will apply clinical expertise in such a manner as to upgrade patient care where indicated and assist the provider in formulating an appropriate Respiratory Care Plan based upon patient assessment, history, lung auscultation, chest x-ray, pulse-oximetry, Respiratory Patient Protocols, etc. The Therapist will communicate changes in the Respiratory Care Plan and/or patient's status to other health care team members as appropriate. Additionally, the Therapist will make recommendations to providers regarding changes in treatment based upon patient observation and diagnostic test values. The Therapist will provide effective and relevant education to the patient, family, students, and co-workers when appropriate.

    The Therapist may perform both fundamental and advanced diagnostic procedures, including pulse oximetry, arterial puncture, blood gas analysis, and Pulmonary Function testing/studies as necessary. The Therapist will perform both emergency and acute Respiratory Care procedures in accordance with departmental guidelines and industry standards.

    Qualifications

    To qualify for this position, applicants must meet all requirements by the closing date of this announcement, 11/27/2019.

    Basic Requirements:

    • United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
    • Have a certificate as an RRT from the NBRC (National Board for Respiratory Care) and a registry number, or a certificate from another body which the NBRC recognizes as its credentialing equivalent.
    • Licensed as a Polysomnographer Technician.

    Preferred Experience: Have a certificate as an RRT from the NBRC (National Board for Respiratory Care) and a registry number, or a certificate from another body which the NBRC recognizes as its credentialing equivalent and be licensed as a Polysomnographer Technician.

    Grade Determinations:

    GS-7. None beyond the basic requirements.

    GS-8. Candidates must have all of the following:

    (1) At least 1 year of successful experience related to respiratory therapy at the GS-7 level or equivalent. Qualifying experience at this level is work which includes duties such as: administering assisted and controlled ventilation to patients with tracheotomies and other complex medical problems requiring frequent adjustments in ventilator parameters; administering and monitoring advanced ventilator techniques such as positive end expiratory pressure and continuous positive airway pressure; developing plans for weaning patients from ventilators; assessing the respiratory status of patients using data acquired through physical observation and clinical analysis of blood gas data, chest x-rays, and electrocardiogram to determine the effectiveness of therapy being administered and to make recommendations to physicians regarding changes in treatment; performing the most difficult respiratory therapy procedures to maintain ventilation, including endotracheal intubation, tracheal lavage, and tracheotomy care; and providing in-service training to others in various specialized areas of respiratory care.

    (2) Demonstrated knowledge and ability needed to perform complex respiratory procedures with minimal supervision. Is able to participate with physicians and nurses and other staff in planning respiratory treatment, with great reliance on the individual's knowledge of the equipment.

    (3) Demonstrated knowledge and ability to provide emergency or critical respiratory care; service as a shift leader or supervisor or sole responsible respiratory therapist on a shift; and to plan and conduct training sessions with respiratory therapy students, hospital staff, patients, and family members.

    Physical Requirements: The tasks of this position generally require sitting, standing, occasional crouching or kneeling. Moderate lifting is required to move equipment throughout the Sleep Lab and Medical Center. Considerable manual dexterity and hand eye coordination are continuously required during polysomnography and patient hookup procedures. This

    includes shift work and continuous observation of computer screen. Cardiopulmonary Resuscitation demands coordination and stamina.

    Work Environment: The work is performed in a sleep disorders lab suite, which includes patient rooms and control room which are relatively small and crowded. Due to the nature of the hospital patient, there is always a possibility of Sleep Technologist contracting infectious disease from patients.
