JOB SUMMARY

The CNA/Unit Coordinator (CNA/UC) provides organizational and administrative support to patient care units and the healthcare team under the direction and supervision of the Nursing Team Leader or other Registered Nurse in charge of the team and/or unit as well as fulfilling the role of CNA, as assigned. The CNA/UC helps to monitor and facilitate activities at the unit nursing station and performs tasks necessary to maintain proper flow of patient care and related documentation. The CNA/UC supports the healthcare team by anticipating and responding to team requests and patient needs. The CNA/UC implements directions form the nursing staff and physicians assisting with direct and indirect patient care; assists patients with activities of daily living, provides basic nursing care to patients and assists in the maintenance of a safe and clean environment. The CNA/UC carries out all aspects of the position using good judgment, problem solving skills, and interpersonal skills. The CNA/UC acts as receptionist and must be able to remain professional with a positive attitude under stressful conditions with multiple demands.

ESSENTIAL JOB FUNCTIONS:

  • Performs Unit Coordinator duties including admissions and unit clerk functions, to include, but no limited to, the following:
  • Interviewing patients or their representative, to obtain personal information or verify information already on file, including emergency numbers and insurance information.
  • Generating appropriate paperwork on each admission type and in accordance with the requirements of the various ancillary departments.
  • Accepting patients for hospital service, both on an in-patient and outpatient basis. Insures appropriate physician documentation is obtained prior to processing patients.
  • Obtaining signatures on Conditions of Admission and initiating the advance directive process, updates status of patient's advance directives.
  • Providing information to the patient/representative about billing, complaint process, patient rights and visiting hours.
  • Approaches family and visitors in a kind and professional manner.
  • Provides explanations and verbal reassurances consistently to family and visitors.
  • Interacts professionally with co-workers and other healthcare providers.
  • Fosters communication between multiple disciplines on behalf of the patient.
  • Participates in informal conferences involving the care of patients.
  • Communicates ideas for improvement to the unit manager/charge nurse.
  • Adheres to SMH Culture of Kindness First Standards.
  • Organizes and manages activities to show consideration of the unit as a whole.
  • Is aware of patient condition and whereabouts when patient is out of unit.
  • Review orders within EMR hourly to ensure X-rays, labs, etc. are completed.
  • Transfers calls to RN within appropriate time from lab with critical results.
  • Makes referral phone calls and follow-up appointments per Physician request.
  • Assists in recording information for performance improvement activities.
  • Assists other staff members as needed with transferring and transporting patient.
  • Demonstrates proficiency in use of computer system.
  • May be assigned special administrative projects by nurse manager.

POSITION QUALIFICATIONS

Education / Experience / License and Certifications

  • High school diploma or general equivalency diploma (GED) required.
  • Current unrestricted Wyoming Certified Nurse Assistant (CNA) License required.
  • BLS certification required.
  • Minimum six months’ related work experience, preferred.
  • Additional certifications may be required based on unit assignment.

Department Specific Education / Experience / License and Certifications

  • NVCI certification within 1 year of position start date for all incumbents as of 9/1/19, NVCI is required by 9/1/20.
