Summary

Provides direct and indirect patient care in the medical-surgical-transitional care setting. Communicates with physician/Manager/co-workers, as appropriate about changes in patient’s clinical condition including results of diagnostic studies and symptomatology. Is able to respond quickly and accurately to changes in condition or response to treatment. Additionally, is able to perform general nursing duties in the departments with adequate supervision. MDS Coordinator facilitates and obtains physician documentation for any clinical conditions or procedures to support the appropriate severity of illness, expected risk of mortality, and complexity of care of the patient. Educates members of the patient care team regarding documentation guidelines and regulatory requirements, including attending physicians, allied health practitioners, nursing, and case management. Supports timely, accurate, and complete documentation of clinical information.

Essential Duties/Responsibilities

  • Completing accurate assessments, MDS & care plans as assigned.
  • Initiating care plans and supporting activities as assigned.
  • Creating and distributing monthly care plan calendars in a timely fashion.
  • Maintaining & updating all care plans and assessments as required.
  • Monitoring & auditing clinical records, ensuring accuracy & timeliness.
  • Informing DON of persistent issues related to non-compliant documentation.
  • Protecting the confidentiality of Resident & Facility information at all times.
Education Qualifications

  • Current unrestricted Wyoming Registered Nurse License.
  • BLS certification, required.
  • At least one (1) years of MDS Coordinator experience, preferred. One nursing experience with in a long-term care center preferred.
  • Central Registry, required.

Additional Skills

  • Must be highly organized, professional and motivated
  • Able to communicate effectively in English, both verbally and in writing
  • Excellent communication skills
  • Basic computer knowledge
  • Additional languages desirable
