JOB SUMMARY

Primary oversight of the patient safety program and responsibility for strategic deployment of patient safety initiatives. Plans, implements, and manages patient safety initiatives that decrease risk of safety events based on incident reporting and issues that arise through ongoing monitoring and reporting. Facilitates process improvement that will support the reduction of medical/health care errors and other factors that contribute to adverse patient outcomes.

ESSENTIAL DUTIES/RESPONSIBILITIES

  • Develops, reviews, and updates annually the Patient Safety Program. Serves as key leader and facilitator of the Patient Safety Committee.
  • Responsible for the coordination, implementation, and evaluation of patient safety initiatives and harm-reduction activities. Recommends appropriate actions based on the analysis to the Patient Safety Committee.
  • Ensure timely investigation of patient safety issues. Oversees / facilitates root cause analysis, apparent cause and common cause analysis investigations; Identifies opportunities for appropriate problem definition, process analysis, reporting and re-measurement and implements appropriate actions based on the analyses.
  • Designs and implements educational presentations that facilitate understanding and implementation of patient safety standards. Trains all new team members in error prevention techniques and provide opportunities for unit specific patient safety education. Collaborates with Unit Managers to review and edit annual education for all team members and monitors compliance.
  • Responsible for the continued strengthening of SMH’s organizational culture of patient safety through integration of Patient Safety initiatives and promotion of Patient Safety culture and teamwork.
  • Collects and presents data and analysis of patient safety trends to hospital committees, medical staff, and Board as needed or requested.
  • Manages the occurrence reporting system, RL Solutions.
  • Promotes safety programs through presentation, staff education and education materials.
  • Maintains accurate records of safety events and mitigation actions.
  • Uses continuous process improvement methodologies to guide patient safety improvement projects and processes redesign by leading, organizing and mentoring improvement teams.

MINIMUM REQUIREMENTS

  • Bachelor Degree in a related field required, Masters preferred.
  • CPHRM or the ability to obtain within 3 years of employment.
  • Current unrestricted RN license, strongly preferred.
  • 2+ years of experience in a clinical setting required.
  • 1 year healthcare Quality Improvement activities preferred.
  • 1 year risk management experience preferred.
  • Yellow belt training or ability to obtain within 6 months of employment.
