JOB SUMMARY

Provides direct patient care in the cardiac and pulmonary rehabilitation departments. Responsibilities involve: the education of patients and their family about cardiac and/or pulmonary disease including, medications, diet, the rehabilitation process and patient goals; patient assessment including, changes in clinical/emotional condition, laboratory tests, diagnostic studies, symptoms, and response to rehabilitation; monitoring of patients during exercise for cardiac and pulmonary abnormalities; scheduling of patient appointments, and other duties as assigned. Responds quickly and accurately to changes in patient condition or response to treatment. Participates in policy and procedure development, performance improvement, and QA activities. Work hours may be irregular.

ESSENTIAL DUTIES/RESPONSIBILITIES

  • Telemetry: identification of dysrhythmias and appropriate reaction based upon department / hospital policies, and ACLS protocol.
  • Assessment of patients’ clinical and emotional status and appropriate response.
  • Assessment and reassessment of pain. Utilizes appropriate pain management techniques. Educates the patient and family regarding pain management.
  • Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.
  • Current knowledge of medications and their correct administration based on age of the patient and clinical condition. Follows the five medication rights and reduces the potential for medication errors.
  • Formulates a teaching plan based upon identified learning needs and evaluates effectiveness of learning, family is included in teaching as appropriate.
  • Interacts professionally with patient/family and involves patient/family in the understanding of the patient’s disease process and rehabilitation.
  • Identifies and addresses psychosocial needs of patients and families
  • Educates patient and family about cardiac and or pulmonary risk factors.
  • Treats patients and their families with respect and dignity.
  • Communicates appropriately and clearly to coworkers and physicians.
  • Consults other departments as appropriate to provide an interdisciplinary approach to the patient’s care.
  • Adapts to change, organized and functions well in stressful situations.
  • Maintains good working relationships, both within the department and with other departments.
  • Formulates a teaching plan based on identified learning needs of a patient and evaluates effectiveness of learning. Patient’s family is included in teaching as appropriate.
  • Performs and documents at least one Performance Improvement activity, annually.
  • Ensures the continuing review of department(s) policies and procedures.
  • Utilizes initiative, strives to maintain steady level of productivity, self-motivated. Completes projects by due date. Documentation meets current standards and policies.
MINIMUM REQUIREMENTS

Education/Experience/Licenses

  • Current Wyoming Registered Nurse License .
  • EKG Part I and Part II (Health Net) course completion.
  • ACLS and BLS certification
Additional Skills

  • Ability to effectively communicate in English, both reading and writing.
  • Basic computer knowledge.
  • Thorough Clinical Assessment skills
