JOB SUMMARY

The Welch Cancer Center Oncology Nurse Navigator is responsible for the coordination, development, and collaboration of programs to be utilized by patients within the organization and community.

ESSENTIAL DUTIES/RESPONSIBILITIES

  • Assist with dietary education and navigate resources to provide patient with needed dietary information.
  • Assist in cross coverage, filling staffing gaps in the chemotherapy and radiation area.
  • Develop Survivorship monthly classes.
  • Develop Survivorship Program (6 week of classes and ongoing care of patient survivors).
  • Revise, coordinate and lead A Shared Journey Support Group.
  • Facilitate “A Beautiful Journey” Class.
  • Provide clinical review of all referrals to streamline patient care to create a more efficient workflow.
  • Assess patient and family needs and coordinate referrals as needed.
  • Maintain a list of local, regional, national resources and manage patient needs.
  • Assist with coordination of appointments, including testing and obtaining results for providers.
  • Work closely with providers on care plans and coordinate follow up care.
  • Serve as primary contact/advocate for patient and families to coordinate care needs between patient, family and caregivers.
  • Assess, and assist if needed, patient’s understanding of disease process and treatment options based on provider recommendations.
  • Provide comprehensive documentation of patient encounters, services, and referrals.
  • Provide and reinforce education in all phases of cancer continuum.
  • Assist with post-treatment transition (i.e survivorship care planning development and follow-up plans).
  • Educate patient, family, caregivers, and the community about cancer care.
  • Educate multidisciplined colleagues about the role of the navigator for resource utilization.
  • Identify barriers, available resources and coordination of care goals with patient.
  • Develop oncology-related education materials.
  • Serve as community resource for cancer programs.
  • Participate in tracking and monitoring metrics and outcomes, and report to leadership.
  • Continue to monitor best practices within cancer center.
  • Implement and measure quality improvement initiatives.
  • Monitor and be a resource for the breast boutique.
  • Participate in community cancer awareness initiatives.
  • Ensure implementation of policies and practices within cancer center.
  • Assist out-of-state patients in coordination of lodging during referred treatments, with the use of Welch Comfort Care Funds in coordination with WCC Manager.
  • Ensures patient information is treated in compliance with current healthcare regulations, medical laws and ethics.
  • Other functions as directed by the Manager.

MINIMUM REQUIREMENTS

  • Associates degree in nursing required, Bachelor’s degree in nursing preferred.
  • Current WY Nursing License without restrictions, required.
  • Current BLS certification, required.
  • Oncology Certified Nurse required or obtained in alignment with ONS Standards.
  • ONS/ONCC Chemotherapy Biotherapy certificate course with current chemotherapy provider card required or willingness to obtain in 90 days.
  • 3-5 years clinical oncology experience, preferred,
  • Knowledge and experience in both radiation oncology and medical oncology, preferred,
