JOB SUMMARY

The licensed practical nurse is responsible for the assessment, coordination, implementation, and evaluation of the delivery of safe and effective patient-centered care in a variety of settings based on education, knowledge, skill set, and competency. The licensed practical nurse communicates with physicians/Managers/co-workers, as appropriate about changes in patient’s clinical condition including results of diagnostic studies and symptomatology. Determines priorities of care based on physical and psychosocial needs. Is able to respond quickly and accurately to changes in condition or response to treatment. Additionally, is able to perform general nursing duties in the departments with adequate supervision. Participates in performance improvement activities.

ESSENTIAL DUTIES/RESPONSIBILITIES

  • Administers all medications using appropriate safety precautions and personal protective equipment (PPE).
  • Prepares cancer patients, both physiologically and psychologically, for chemotherapy.
  • Instructs patients and families regarding their schedule and acts as a liaison for coordination of appointments across departments.
  • Coordinates and schedules patient care maintaining visual management boards for tracking new orders and medications for appropriate intervals of administration.
  • Triages patient calls and educates patient as appropriate to plan of care. Escalation of calls to appropriate provider when necessary.
  • Tracking of prior authorizations and approval or denials of medications in patient care plan. Works collaboratively with financial advocates, prior authorization staff and pharmacy to procure medications for patients timely and within patient scheduled time frames.
  • Assists with gathering information for coding or billing to assist with reimbursement of procedures.
  • Ensures balance of schedules across providers, nursing and pharmacy staff utilizing acuity tools.
  • Assists with medication refills.
  • Assists with order entry to assist provider.
  • Maintains clear communication with other offices on the scheduling of external appointments.

MINIMUM REQUIREMENTS

Education / Experience / License and Certifications

Current Licensed Practical Nursing license in the State of Wyoming without restrictions.

BLS certification, required.

Additional certifications may be required based on unit assignment.

Minimum of one year of experience, required.

  • Able to communicate effectively in English, both verbally and in writing

  • Participates in continuing education activities within the department and organization
  • Attends and actively participates in departmental staff meetings
  • Keeps up to date with clinical practice and evidence-based practice guidelines
  • Stays abreast of organizational policies and procedures
