Summary/Objective

The Housing Retention Coordinator is responsible for providing best practices in case management to improve the retention of housing by Veterans who have previously participated in a Veteran Homeless Program such at the Veteran Administration’s (VA) Homeless Providers Grant and Per Diem (GPD) Program or the VA’s Healthcare for Homeless Veterans (HCHV), or Contracted Residential Services (CRS).

Essential Functions

  • Provide best practices in case management to help qualified Veterans maintain housing and long-term stability. This will include but not limited to:

  • Making home visits to Veteran’s homes to monitor housing stability
  • Provide and/or coordinate educational activities related to meal planning, tenant responsibilities, the use of public transportation, community resources, financial management, and the development of natural supports in their communities, including moral injury
  • Making referrals to needed services, such as mental health, substance use disorder, medical and employment services
  • Participating in case conferencing with other service providers who are working with the Veterans
  • Develop client-centered, individual care plans with goals that address barriers to self-sufficiency
  • Cultivate relationships with appropriate community resources to ensure necessary referrals for services pertinent to Veterans long term stability
  • Enter data into identified data systems in your respective coverage area and provide reports as contractually obligated and collaboration with Continuums of Care to provide quality, timely, and accurate information regarding any needs of the Veterans.

Supervisory Responsibility

This position has no direct supervisory responsibilities.

This job operates in an office environment, Veteran homes, and community settings.

While performing the duties of this job, the employee is regularly required to talk or hear. Specific vision abilities required by this job include close vision and ability to adjust focus.

Position Type/Expected Hours of Work

This is a part-time position with a maximum of 28-30 hours a week as assigned by the program manager/supervisor of this program.

Travel

The Housing Retention Coordinator must be willing to routinely travel as necessary to provide quality case management to Veterans in their homes, and to and from community partner meetings to provide collaborative and best-practices in case management.

Required Education, Experience or Eligibility Qualifications

  • An associate degree with a minimum of two years of case management experience or related experience

- OR-

  • Degree plus applicable and adequate experience in the fields of human service, Veterans Affairs, or administration

  • Valid driver’s license and successful federal criminal background check with approved status
  • Other qualifications and experience will be taken into consideration for current VOA employees applying for this position

Preferred Education

  • Bachelor’s Degree in human services, behavioral science or related field with one year of case management experience

  • Preference will be given to Veterans
