About the Role:
We’re seeking an Outreach Representative to help expand our network of healthcare partners across San Bernardino County. This is a field-based, commission role ideal for someone with experience in healthcare marketing, referrals, or provider relations.
Responsibilities:
Outreach to hospitals, skilled nursing facilities, home health agencies, and ECM providers and invite them to visit our facility, drop off brochures, offer our services to them.
Build and maintain professional relationships with discharge planners, case managers, and referral coordinators
Coordinate introductions and help establish formal referral partnerships
Compensation:
$250 per successful partnership
Bonus opportunities for high performance
Ideal Candidate:
Experience in healthcare outreach, hospital liaison, or marketing/referral development
Strong communication and relationship-building skills
Reliable transportation and professional appearance
Self-motivated and able to work independently
