OVERVIEW: The Social Worker acts as the liaison between the child and all other parties significant in his/her life such as the County Worker, attorney, Therapist, Mental Health Specialist, school, family, CASA, friends, and all Residential staff members. This position serves as Head of Service for the STRTP. The Social Worker will work with the child in a Trauma Informed manner to help the child and staff members understand the child’s triggers and needs. The end goal is to help the child with emotional regulation and behavioral safety so he/she can return to a family setting.

DUTIES:

Social Work Activities

Ensures that the team delivers appropriate services that are compliant with SCH standards of care and established resident’s goals.

Represents the current treatment and behavioral intervention plan and its intent at provider meetings including Care Review, Child and Family Team (CFT), Individual Education Plan (IEP), Student Study Team (SST) and any other identified needs.

Facilitate CFT meetings and represent resident and family at CFT meetings, Care Review and other community coordination needs as necessary.

Acts as primary point of contact for all residential-based services communication with the county placement workers.

Ensures the cottage integration of skill-based and behavior-based programs for the residents (i.e. Trauma Informed Care and Love and Logic, etc.) and serves as a leader for cottage staff by role modeling these skills.

Assists in training cottage staff to address the individual needs of their residents, in conjunction with the Cottage Supervisor, Lead Residential Counselor and Mental Health Specialist.

Ensures compliance with Community Care Licensing (CCL) residential program responsibilities by completing appropriate documentation and communication protocols with residents’ family, natural supports, placement worker and licensing agency as necessary.

Completes Needs and Services Plan every 30 days.

Coordinates with child’s school, Residential Mental Health Supervisor and (as needed) Training and Therapy Recreation Manager to advocate for residents’ educational rights and assure that residents are in school and appropriately placed.

Conducts group and individual rehabilitation sessions for residents.

Meets monthly productivity expectations.

Completes and submits billing notes within established time frames.

Maintains a safe, healthy and therapeutic environment at the STRTP

Ensures that each child admitted to the program has a mental health assessment

Ensures that each child in the STRTP has commonality of needs with the other children in the STRTP, including whether the child’s presence is adverse to the safety of mental health needs of the child or other children admitted to the STRTP

Ensures the mental health services identified on each treatment plan are provided and appropriate to meet the individual needs of the child

Makes arrangements, including transportation, for children to receive mental health services that cannot be provided by the STRTP

Arranges for special provision of mental health services to children with disabilities including visual and auditory impairment

QUALIFICATIONS: Graduating with a Master’s Degree in a Behavioral Science and registered with the California Board of Behavioral Sciences as a LCSW (ACSW), MFT (AMFT) or LPPC (APCC).

New graduates may be considered for employment and are required to be registered with the BBS within 90 days of hire. Students may be considered for employment as long as they are within 60 days of graduation with an applicable Master’s degree and are required to be registered with the BBS by 90 days of graduation. Unregistered graduates and students are required to have at least 2 years of qualified mental health experience.

Must receive LMFT, LCSW, or LPCC license within six years of initial registration with the California BBS as an intern to meet Sacramento County contract requirements.

Experience working in a residential setting is desired. Prior experience with children, adolescents and families with emotional problems is preferred.

Driving is an essential function of this job. As such, employees in this position will be required to furnish their own vehicle to be used in the course of employment. A valid driver’s license and minimum insurance coverage is also required. Must be at least 21 years old.
