Position Summary:
We are seeking Host Home Providers to share their home with adults who need ongoing care and support in a family-style setting. Providers offer personal care, daily assistance, and inclusion in community and home life.
Responsibilities:
☑️ Provide personal care, meals, and hygiene assistance
☑️ Assist with medication administration and medical appointments
☑️ Provide transportation and community engagement
☑️ Complete daily and monthly documentation
☑️ Maintain a safe, positive home environment
☑️ Attend monthly agency check-ins and training
☑️ Communicate regularly with the agency team
