The Transitional Care Clinical Liaison is responsible for the growth of
new and existing referral sources in assigned market for CCVNA service
lines. Serve as SME regarding CCVNA transitions of care, responsible for
management and completion of referrals. Ensures compliance with
standards of CCHC, VNA and regulations set by all accrediting, licensing
and regulatory organizations. Description Provide comprehensive clinical
assessment /screening of patients for appropriate levels of homecare.
Completes 80 - 100 referrals a month. Responsible for the collection and
transfer of appropriate clinical documentation to ensure seamless
transitions of care. Promotion of complete agency service line within
assigned coverage area as directed. Maintains and develop knowledge base
regarding regulatory guidelines for homecare. Reports on competitor
activity and conducts competitive analysis.Serves a resource and point
of contact for referral sources and patients. Maintains working
knowledge of e-discharge and Suture Sign platforms. Maintenance and
development of CRM. Submittal of weekly activity log. Other duties as
assigned. Consistently provides service excellence to all patients,
family members, visitors, volunteers and co-workers in a manner that
reflects VNA?s commitment to CARES: compassion, accountability, respect,
excellence and service. Qualifications LPN, RN, BSN preferred. 3 years
clinical liaison experience, homecare/hospice preferred. Ability to
develop and maintain new business relationships with referral sources,
patients and families. Proven sales track record within healthcare
environment. Familiarity with E-Discharge and CRM platforms. Knowledge
of palliative/oncology/chronic disease pathways. Schedule Details: 37.5
Hrs. Per week, Monday-Friday, 8a-4p, Occ. weekends and holidays
