Req Number 134629Job DescriptionPerforms an age specific plan of care for designated group of residents using the nursing process of assessment, diagnosis, outcome identification, planning, implementation, and evaluation of resident care. Collaborates with physicians and other team members in coordinating and implementing procedures and treatments. Uses leadership skills and clinical judgment in coordinating resident care and directing and delegating activities of the resident care team, specifically the LPN, CAN and Unit Secretary.Job Responsibility
Utilizes the nursing process in giving care to assigned residents.Performs comprehensive physical and psychosocial assessment.Utilizes assessment data to determine resident's actual or potential problems.Chooses and plans for appropriate proper nursing interventions and adjust care, if appropriate.Collaborates with physicians and other health care team members in developing and implementing plan of care.Evaluates resident's responses to interventions.Administers prescribed medications correctly.Determines medications to be administered, its actions, indications, therapeutic effects, adverse reactions, etc.Administers medications timely, accurately, and consistently as per protocol.Uses knowledge in the administration of medications based on a resident's age.Participates in the education of resident and/or significant other.Explains procedure and intervention to resident as indicated/requested.Recognizes and utilizes health teaching opportunities and resources/ materials available for teaching.Assesses the effectiveness of resident teaching by feedback from resident and family.Participates in interdisciplinary discharge planning activities.Identifies discharge planning activities.Recognizes and utilizes health teaching opportunities and resources/ materials available for teaching.Initiates resident education in preparation for discharge.Follows departmental policy with documentation.Documents comprehensive assessment and reassessment of residents' needs within prescribed time frame.Utilizes care plan and writes progress notes to reflect residents' status accurately.Uses appropriate methods of documentation according to departmental policy.Performs related duties, as required.ADA Essential Functions
Graduate from an accredited School of Nursing. Bachelor's Degree in Nursing, preferred. Must be enrolled in an accredited program within 24 months of employment, if hired after September 1, 2010 and obtain a BSN degree within five (5) years of employment date. Current license to practice as a Registered Professional Nurse in New York State.BLS Certification.One year previous experience including charge nurse, Minimum Data Sets (MDS) and Patient Review Instruments (PRI).
