Clinical outcomes are equal to or better than inpatient hospitalization. Work in a team environment (Physicians, NPs, RNs) in a virtual Command Center Medically Home significantly reduces total medicine expense from avoidable inpatient visits, post-acute care and readmissions. Improved patient experience. State of the art equipment at Command Center and patients’ home. Patients have home infusion, meals/nutrient consults, pharmacy/medical mgmt., mobile imaging, laboratory and DME/oxygen. Integrated care with primary care providers, specialists & ancillary departments.Minimum Qualifications:Licensure, Certification, Registration: BC/BE Internal Medicine/Family Medicine/Geriatrics.Skills, Knowledge & Abilities: Experience as a hospitalist or SNF a plus. Committed to providing high quality, evidenced based medicine to a diverse patient population. Excellent clinical and communication skills.
