340B Medication Adherence SpecialistJob DetailsJob LocationAdelante Healthcare Center Support Office - Phoenix, AZ Position TypeFull Time DescriptionPOSITION SUMMARY The 340B Medication Adherence Specialist is responsible for contributing to Adelante’s Value Based Care (VBC) measures. This includes but is not limited to participating in related projects/initiatives, compliance, auditing, and medication management related to the health plan metrics five-star ratings for Adelante Healthcare. QualificationsESSENTIAL SKILLS AND EXPERIENCE:

High school diploma or GED from an accredited institution

Certified Pharmacy Technician and/or Certified Medical Assistant, required. Certified Pharmacy Technicians must be in good standing with Arizona State Board of Pharmacy and remain so for their duration of employment with Adelante Healthcare.

Three (3) plus years of medication management experience.

Two (2) plus years of experience in a healthcare or pharmacy setting.

340B experience strongly preferred.

Excel experience required.

Strong analytical skills with attention to detail and the ability to translate data into a quality analysis.

Self-motivated, action-oriented, strong drive with the ability to make decisions, act independently, and meet deadlines.

Medication insurance billing and medication claim submission experience.

Certification to perform cardiopulmonary Resuscitation for the Health Care Professional (CPR) and AED through courses that follow the guidelines from the American Heart Association and Red Cross (cognitive and skills evaluations).

Valid Level One Fingerprint Clearance Card issued by the Arizona Department of Public Safety for all specialty behavioral health locations.

POSITION REPONSIBILITIES

Achieve therapeutic goals and improving patient outcomes.

Assist patients with barriers to care.

Communicate with pharmacy staff and providers on patient outcomes.

Prioritize patients’ immediate needs.

Maintain health plan metrics criteria in portals.

Analyze spreadsheets and patient data.

Provide support in medication adherence opportunities as well as other pharmacy related needs.

Prioritize converting prescriptions to 90 or 100-day supply.

Monitor single fills to help anticipate when next fill is due.

Experience working in a Federally Qualified Health Center

Work environment: While performing the duties of this job, the employee is exposed to weather conditions prevalent at the time when traveling to various clinic sites. The noise level in the work environment is usually moderate. The employee may be subject to health hazards (contagious diseases, blood borne pathogens, etc.) when working in the clinic area.
