The Long-Term Acute Care Registered Nurse provides planning and delivery of direct and indirect patient care to medically complex patients in long-term acute settings. Essential Duties and Responsibilities:

Provides planning and delivery of direct and indirect patient care

Responsible for providing care to medically complex patients

Supervises care, treatment, and services of the patient(s)

Documents the patient’s plan of care using identified nursing diagnosis, expected patient outcomes and selected nursing interventions

Maintains confidentiality at all times with patients, medical staff and co-workers

Provides a safe, comfortable and therapeutic patient environment

Adheres to and supports the policies/procedures/goals/objectives of the hospital in an attempt to provide quality patient care

Revises plan of care according to evaluation, changes in plan of care and nursing interventions

Performs interventions according to identified priorities, plan of care, and facility policies and patient care outcome standards

Identifies and prioritizes actions based upon patient care requirements

Minimum Requirements:

Current RN license for the state in which the nurse practices in

One (1) year nursing experience in a long-term acute care facility preferred

Experience in acute rehabilitation for vents, wound care, cardiac, and stable post trauma patients

Complies with all relevant professional standards of practice

Participation and completion of Amergis' Competency program when applicable

Current CPR if applicable

TB questionnaire, PPD or chest x-ray if applicable

Current Health certificate (per contract or state regulation)

Must meet all federal, state and local requirements

Successful completion of new hire training as applicable to job site

Understand patient confidentiality and HIPAA requirements

Ability to effectively elicit/provide information to and from appropriate individuals (including, but not limited to, supervisors, co-workers, clients) via strong communication skills; proficiency in the English language is required

Computer proficiency required

Must be at least 18 years of age
