Along with physicians and other members of your rehab team you will go along on the patients’ journey providing care and education so they can resume their lives at home with their families.You’ll use a wide variety of nursing skills in this position. Your day-to-day nursing tasks will depend on the tailored needs of your patients. Some may need direct acute care; some may not. In addition to monitoring you’ll also educate patients and their families on how to continue quality care once they’re at home.To thrive in this role you need a wide range of nursing skills. You need to be upbeat as our patients have a long road ahead of them and sometimes need a boost. Most importantly you’ll advocate for their needs and wellbeing by ensuring they have the resources and support needed at discharge. Carefully monitor patients for changes in neurological status or other conditions
Prepare patients for various speech physical occupational or respiratory therapies they have scheduled each day.
Educate the patient and caregivers on how to continue certain care at home.
Identify opportunities for improvement in care plans and on the rehab unit

Current license as a Registered Nurse in the State of California.
Minimum one-year Acute Care experience
Current basic life support (BLS) certification
