The Family, Youth, and Children Social Service Worker II (SSW II) will be assigned to the Valley of the Moon Children’s Center’s newly created Short-Term Residential Therapeutic Program (STRTP), which provides services to meet the behavioral and mental health needs of Sonoma County’s foster youth. Additional responsibilities include:
Providing intake coordination for youth entering the Short-Term Residential Therapeutic Program (STRTP)
Reviewing and processing STRTP referrals and supporting documents, and initiating clinical referral review
Interviewing clients to gather data to develop a case and provide a basis for the evaluation of admission into the STRTP
Assessing and evaluating information from referral documents and initial program interview(s) to establish Needs and Services plans
Coordinating with supervisors, clinicians, service providers, youth counselors, foster parents, biological family, and other health partners/agencies to update Needs and Services plans and ensure youth continuity of care
Coordinating and facilitating monthly meetings with clinicians, program staff, social workers, and family members to discuss youth progress
Providing service navigation to assist youth in addressing educational, behavioral, and medical needs
Developing, implementing, and coordinating agency and community resources to meet social service needs
Making home visits to support youth transitioning into a home-based setting
Transporting youth to and from medical appointments
Preparing and maintaining detailed and concise written case records, reports, forms, and correspondence in order to satisfy county, state, and federal requirements
The ideal candidate will possess:
Education in child development, coaching, training, parent education, learning theory, psychology, mental health, substance use disorders, trauma informed care, and/or social services
Experience working with children, youth, and adults
Excellent communication, multitasking, and problem-solving skills
The ability to keep calm in a stressful work environment
Experience working with individuals with mental health issues, life instability, actively using/abusing substances, and/or in early substance use recovery
Knowledge of trauma-informed care approaches, strength-based assessments, and cultural humility
Experience in a fast paced, dynamic work setting managing multiple competing work priorities
Experience working with underserved multicultural communities and diverse populations
Basic Microsoft Office and data entry skills
Bilingual (English/Spanish) speaking abilities are highly desirable but not required
