ALS Caregiver (RN/LVN or equivalent ALS experience)for ALS Morning & Evening Care — Oakland (Lake Merritt Area)

Hiring multiple caregivers for 7-day coverage $30 training rate → $45/hr once fully trained Live-in Option available (less hourly).

About Us

We are a family in Oakland (Lake Merritt area) supporting Casey — a climate strategist and an active participant in the BrainGate brain-computer interface (BCI) clinical trial. Learn more about him here: https://linktr.ee/caseyharrell. Casey has advanced (but very stable) ALS and requires consistent morning and evening skilled care. Our home is grounded in disability justice; autonomy; compassion; and calm, relationship-based caregiving.

Casey’s spouse (primary caregiver transitioning out of 24/7 care after 7 years)

An active community of care and researchers

Your role is essential to ensuring Casey can get up safely, start his day with stability, and/or get settled comfortably at night.

We are hiring multiple RNs/LVNs to create a sustainable 7-day schedule.

Position DetailsShift Structure

We are hiring 2–4 caregivers to cover:

Morning shifts: 3–5 hours typically 7:00 AM - 10:00 AM

Evening shifts: 5–8 hours typically 5:00 PM - 12:00 AM

You may take only AM, only PM, or both. or more hours as a live-in at $25-$30

We work together to build a balanced weekly schedule with backup coverage.

Compensation

Training rate: $30/hour (typically 2–4 weeks depending on comfort/competency)

Fully trained caregiver rate: $45/hour

Additional hours available for specialized tasks

Can pay through a combination of private pay + IHSS, W2 option also available

Responsibilities

You will provide skilled morning and/or evening care, including:

Morning routine (AM)

Physical Therapy

Catheter support

Hygiene (washing, advanced oral care, dressing)

Transfer with a ceiling lift

Support with respiratory equipment (Astral ventilator; training provided)

G-tube equipment and basic prep

Suctioning as needed

Supportive communication with daytime staff

Evening routine (PM)

Respiratory Physical Therapy (Vest, Cough Assist, Nebulizer)

Preparing and administering G-tube food (with pump) and meds (with syringe)

Toileting and catheter support

Transfer with a pivot board or ceiling lift

Setting Casey up for overnight stability

Dishes and laundry related to Casey's care

Teamwork

Coordinate with daytime assistants

Communicate clearly with the live-in support fellow

Manage the time to keep the schedule on track, communicate with others any updates to the schedule and when/if their assistance is needed

Provide concise notes and verbal updates to spouse caregiver

You Are Someone Who

Is an RN or LVN (active license in CA) or has equivalent ALS caregiving experience and open to learning tailored family approaches and preferences.

Has grit, humor, and hustle. Enjoys a challenging and sometimes changing work environment; can respond in emergency situations with a calm demeanor.

Precise and detail oriented in regards to medication management and prep, as well as the myriad micro-body and equipment adjustments an advanced ALS patient requires.

Skilled, pro-active, direct and patient communicator. Can go at Casey’s pace with his speech technologies, and wait for clear confirmed consent and directions before proceeding with anything.

Respects autonomy and works relationally, not clinically

Reliable, wants regular and long-term schedules and can give multiple days notice before taking time off.

(For evening shifts) can stay until Casey is fully ready for bed, has flexibility for occasional later nights.

Is LGBTQ+ friendly and anti-ablist.

Requirements

IHSS certified OR willing to complete the certification process

Able to work at least 2 consistent shifts per week

Comfortable with G-tube and catheter care (or willing to train)

Comfortable with “hands on” toileting support

Comfortable with respiratory equipment (training provided)

Able to lift, transfer, and assist safely with adaptive equipment
