West Marin Senior Services seeks a Care Manager.
Responsibilities: conduct in-person and/or phone interviews, and annual assessments. Develop, implement, and monitor care plans for clients. WMSS Care Managers work to build strong bonds and trust with ongoing evaluations to ensure client success.
Skills & experience: LCSW or RN desirable, 4-year college
degree, background in social work, nursing, or homecare required. Bilingual skills
in Spanish and English are highly desirable. A valid driver's license and cell phone
are required.
Salary & benefits: Flexible 32-40 hours per week depending on
caseload. Hybrid/remote work schedule may be possible after 90 days.
Salary: Based on experience. Benefits: Medical, 401K, paid vacation/sick days,
and mileage reimbursement.
