The Older Adult FSP Case Manager works as part of the FSP team at Wilshire Community Services. The Case Manager will be responsible for working in close collaboration with the FSP Clinician to support clients as they strive to reach their treatment plan goals. The Case Manager provides case management as needed to include: connection to community resources, assistance with transportation, assistance with securing basic needs (food, clothing, and shelter), and providing coaching/education To FSP clients.
The ideal candidate would have experience working with older adults with mental illness. We are looking for an experienced case manager that can work compassionately with that population and will work well within our team that provides wrap around services. 30-40 hours a week with potential extra time for on-call after hours. Flexible schedule allowed.
