Conducting New and existing assessments for clients in Virginia for Medicaid.
Requirements
Needs to be RN
Responsibilities:
Demonstrate proficient skills in assessing potential and current client’s condition
Observe and supervise client care administered by caregivers in accordance with plan of care notes
Maintain compliance with agency policy and procedures
Follow Medicaid regulations
Perform the initial home care patient visit and re-evaluate the patient’s needs and progress on a regular basis
Initiate the plan of care under doctor’s orders
Perform admission, transfer, re-certification, resumption of care, and discharge for the home care patient.
basic computer skill for assessments and re-assessments (willing to train on current software to right candidate)
Educate patients and caregivers on disease process, medications, plan of care, and individualized treatment plans.
Update clinical records according to policy and procedures.
